Description
This is a notice for the procurement of commercial items prepared in accordance with the subparts in the Federal Acquisition Regulation 12.6 and 14. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-01. The solicitation number assigned to this procurement is 673-36-02. The solicitation number shall be referenced on all requests. The North American Classification Code assigned is 811219; the small business size standard is $5.0 million. The James A. Haley Veterans Hospital, Tampa intends to award a firm fixed price contract for a full service preventive maintenance and repair of the PACS/CR and all associated medical equipment stated in the solicitation for a base period of one year, beginning February 1, 2002, or date of Notice to Proceed with option to extend (4) four additional one year periods subject to required need and the availability of funds. A written solicitation will be issued, on January 9, 2002. The contractor will provide all labor, materials, tools, equipment, transportation, parts, shipping and software upgrades required to perform maintenance and repair to the system and all equipment listed.. The solicitation also provides for the inclusion of a full-time field service engineer and training of a VA staff engineer.. The Vendor will provide for shipping costs to and from the VAMC Tampa, FL and associated clinics and will provide suitable shipping containers as required. A pre-proposal conference and site visit is scheduled for January 16, 2002 at 10:00 AM E.S.T. at the J.A.H. VA Hospital, Building #42, RM 217, 2702, 131st Ave., Tampa, FL 33612. Each respondent must have an established business with an office and full time staff to include a fully qualified, competent field-service engineer (FSE) that will be located on site Monday thru Friday (8 AM to 5 PM) and back-up (FSE) , both who have successfully completed a formalized training program for all of the equipment identified including a minimum of two years experience with respect to the scheduled and unscheduled preventive and remedial maintenance of the equipment. Large businesses must submit their Sub-contracting plan in accordance with FAR 219-8 and 219-9 with offering documents. Requests for a solicitation package are to be made in writing and addressed to the attention of the Contract Officer. The government will not be held responsible for the quality of faxes. Telephone requests will NOT be accepted. All offers shall be delivered to the Contracting Officer by 3:00 PM E.S.T. Monday, January 28, 2002 at: James A. Haley Veterans Hospital (90C), 13000 Bruce B.
