Description
NA Contractor agrees to provide in depth inpatient and outpatient coding education for administrative and clinical staff, as well as medical record assessment of documentation and coding in accordance with the ?Work Statement? set forth herein, at the VA Roseburg Healthcare System, Roseburg Oregon (VA). Contractor shall provide all labor, travel, materials and supervision necessary to perform the work stated herein. Services shall be performed Monday through Friday during normal working hours of 8:00 AM ? 4:30PM, excluding Federal Holidays. It is required that contractor personnel providing these services have a minimum of five (5) years of actual coding consultation experience. The contractor shall agree that: All deliverables associated with working papers, and other material deemed relevant by VA generated by the Contractor in the performance of this contract are the property of the Untied States Government. All individually identifiable health records shall be treated with the strictest confidentiality. Access to records shall be limited to essential personnel only. Records shall be secured when not in use. At the conclusion of the contract services all copies of individually identifiable health records shall be destroyed or returned to the VA. The contractor shall comply with the Privacy Act, 38 U.S.C. 7332; reviewers will be required to sign a confidentiality statement upon arrival at the facility in coordination with the COTR. Due to the complexity of this contract action, subcontracting is prohibited. Contractor must provide trainers with current credentials of an Accredited Record Technician (ART) or a Registered Record Administrator (RRA) who are additionally certified in Outpatient Coding such as a Certified Coding Specialist (CCS) or a Certified Coding Specialist-Professional (CCS-P). The actual reviewer must have at least five (5) years of experience and training as a consultant in reviewing hospital records and outpatient health care organizations. Hospital experience must demonstrate a knowledge of coding conventions for subspecialties and primary care, as well as experience in physician education and training. When reviewing outpatient coding, the reviewer must be certified in outpatient coding. References, as well as proof of credentials, must be included with the quotation. Contractor to perform off-site review of ICD-9-CM diagnosis and procedure codes and CPT-4 procedure codes on outpatient claims. Types of outpatient records to be reviewed include but not limited to: same-day surgery, ER and/or clinic visits. These reviews will identify potential opportunities to assure appropriate reimbursement amounts by improving the clinical documentation, coding and billing procedures and continue through the term of the contract to include option years. The review shall include the following: a) PCE data compared with medical record data for: i. SC/NSC designation. ii. ICD-9 and CPT codes correct. iii. Documentation appropriate and supports E and M codes. iv. Documentation properly authenticated. b) Billing Information: i. Did the billing document reflect the codes entered into PCE? ii. If changed was provider notified? iii. Was a bill initiated for an encounter that the provider indicated was SC? c) Analysis of review: i. Financial extrapolation of results. ii. Recommendations for potential appropriate revenue enhancements. Contractor will provide physician-training materials. These materials would describe the process, primary and secondary diagnoses, Evaluation and Management coding, CPT-4 coding, reasonable charges, and the impact of medical record and encounter form documentation on the revenue cycle. Contractor will provide training to clinical providers (physicians, registered nurses, psychologists, physician assistants, social workers, and nurse practitioners), coders and billers. Contractor will conduct training and provide participants with written training tools, including but not limited to: a) Analysis techniques to identify areas for improvement. b) Presentation for physicians. c) Methods for optimization of network resource utilization. d) Evaluation tool for assuring assimilation of training. Training sessions shall consist of twenty (20) hours of coding and documentation training for administrative and clinical staff. Administrative coders training shall consist of in depth inpatient and outpatient coding education, including ICD-9 diagnosis coding, IDC-9 procedure coding, CPT evaluation and management services, and CPT procedures coding use of modifiers. The clinical training will consist of CPT evaluation and management services and appropriate documentation. NAICS Code 541611/$5 million applies.
