Description

Provide employee physicals for the USINS Baltimore District Office located in Baltimore, MD. An estimated 19 employees are expected to participate. Requires a single provider with the capability to perform all tests within a single facility located in close proximity to the requesting office located at 31 Hopkins Plaza, Baltimore, MD 21201. All examinations must be completed by June 30, 2002 and billed by August 1, 2002. Response due date has been extended to 02/21/2002. Facilities must be accessible by public transportation or pedestrian travel from the Baltimore District Office. Please provide evidence of such accessibility. This modification was issued in MOD 1 on 02/14/2002 prior to the response deadline, but for whatever reason the changes are not showing up in the FedBizOpps system, so are being posted again.
