- The United States Attorney's Office in Phoenix, Arizona has a requirement for a contractor to provide Document Management Support Services. The Government intends to enter into Blanket Purchase Agreement with one or more vendors capable of providing these services. The performance period will be a twelve month base year with four twelve-month option period. The exercise of option period is at the discretion of the government. This notice is prepared in accordance with FAR 13, Simplified Acquisition Procedures. Accordingly, the announcement constitutes the Request for Quote(RFQ) and is the only solicitation that will be issued. This Request for quote has a 50 mile geographic restriction to Phoenix, Arizona commuting area. This requirement is a 100% set-aside for a small business with NAICS 541199. The only work that will be authorized under the BPA is work ordered by the government through the issuance of Calls by the Contracting Officer. The government makes no representation as to the number of Calls or actual amounts of work will be ordered. The government requests a firm-fix labor rate for a Base and four year option for the following service. Specifications are stated as follows:The Contractor shall furnish the necessary personnel, material, equipment (except as otherwise specified), to perform the following Statement of Objective. Background The United States Attorneys' Office (USAO) has a requirement for Document and Image Management in the District of Arizona on behalf of the United States Department of Justice. The requirement include copying, bates stamping, imaging, entry into database and trial presentation software. Scope of Work The Contractor shall provide the necessary services to the District of Arizona, as otherwise specified, to perform the following services: Print records on a laser printer rather than a dot matrix printer for clarity and printing separate pages. Use ROI package to leave one-inch margins for hole punching (use 3 hole punched paper). Reduce copies by 10% for hole punch purposes. Bates stamp records in the same order as the originals are kept. Work with USAO Automated Litigation Support (ALS) staff on the imaging, database and trial presentation software to be used, including entry of images into the selected software if requested. Determine whether hard copies or images will be used in discovery and provide enough copies to avoid additional future copying. Skills and Position Description Personnel assigned to perform the services shall be required to possess a diverse set of skills. The following is a list of positions or skill categories which the Contractor is expected to provide. 1)Copying and Imaging 2) knowledge of ACCESS software 3) ability to create load files such as: Sanction, Concordance, Summation, Trial Director also, CaseMap Software.4) data entry capabilities Resumes The USAO requires resumes for all proposed personnel. All resumes must be certified by the Contractor that the information submitted is true and complete. . Personnel resumes shall include the following information: Name, Education and qualifying experience Place of Performance: The BPA's require performance at the office of the United States Attorney Office, District of Arizona, two Renaissance Square, 40 N. Central Avenue, Suite 1200, Phoenix AZ All contract personnel must pass a name and fingerprint check and possibly a full background investigation for security purposed. Evaluation: The government will issue a BPA resulting from the offerors whose proposal is the represent the best value to the government, price and other factors considered.. The following factors will be used to evaluate offers: (A) Past Performance; (B) price and. ( C) Technical Capability. Factors A and C are more important than Factor B. The offerors should provide three recent and relevant past performance with point of contacts, and current telephone number. Offeror's are requested to submit one (1) signed and dated original and two (2) copies of the quote .
