Description
NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation 502-52-02 is a request for proposal. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-06. The NAICS 339112 code is and the small business size standard is 500. VA Medical Center requirements: Service _ Perform any and all required service, repair and calibrations in accordance with Original Equipment Manufacturer recommendations and specifications to include 2,500 hour, 10,000 hour and annual preventive maintenance utilizing new original equipment manufacturer parts and kits on all Puritan Bennett Ventilators on station. This will include all parts and software revisions making all parts, hardware and software components compliant with the most recent manufactured released. The Service Technician who will perform any-and-all work on our equipment must be individually trained and certified by Puritan-Bennett on all models of ventilators on this station. The Service Technician will provide and any and all standard tools, software, necessary for service and repair of the equipment. Before beginning work, all Service Technicians must check in through Engineering Office located in Building 5, Room 109 to sign-in and receive a visitor_s badge. Hours for reporting in will be from Monday through Friday 7:00 a.m. to 4:00 p.m. The Service Technician must contact the Contracting Officers Technician Representative (COTR). After completion all work successfully performed will be signed off by the COTR. The Service Technician or any other representative of the Service Providing Company will not use a cellular phone in any critical patient area (Surgery, Recovery, ICU, ETU and Telemetry) within the VAMC Facility. All signs and labels damaged during servicing of the equipment shall be replaced at the Service Providing Company_s expense. All equipment and parts covered under this agreement are the property of the VAMC and shall not be removed without the express permission the COTR. Equipment Serviced includes the following. Puritan Bennett Corporation Ventilators Model 7200, Serial numbers include 8848035, 9371273, 8847758, 9171705, 9171619, 9171661,9371321, 9371292, 9171541, 9371322, 9171732. Suggested source of this equipment is Mallinckrodt/NPB, 675 McDonnell Boulevard, St. Louis, MO 63134. Offerors responding to this announcement shall provide the information contained in FAR clause 52.212-1, Instructions to Offeror-Commercial Items, and shall include a completed copy of provision 52.212-3, Offeror Representations and certification-Commercial Items. An authorized representative must sign the representations and certifications. The offeror shall also provide its Contractor Identification Number (DUNS), and Tax Identification Number (TIN). Clause 52.212-2, Evaluation-Commercial Items. The Government will award a contract resulting from This solicitation to the responsible offeror whose quote will be most advantageous to the Government, price and other factors considered. Evaluation factors are 1). Price; 2) Technical capability (equal importance). Offer shall submit three references that you have performed a similar or same type service. Clause 52.212-4, Contract Terms and Conditions-Commercial Items. Clause 52.212-5, Contract Terms and Conditions required to implement Statutes or Executive Orders-Commercial Items. Clause 52.228-5, Insurance-Work on a Government Installation. Quotes are due back August 30, 2002 by 2:30 PM central daylight saving time, they should be addressed to the attention of Kathryn W.
