- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will NOT be issued. The purchase request number is F756MG21930400, and is being issued as a sole-source Request for Quotation. The solicitation incorporates provisions and clauses as those in effect through Federal Acquisition Circular 01-05. This procurement is a sole source through GE Medical Systems. The North American Industry Classification System code (NAICS) is 811219, Other electronic and precision equipment repair and maintenance. The size standard of $6,000,000 does not apply for award to sole source large business. DESCRIPTION: X-Ray systems. REQUIREMENTS: Provide all labor, tools, parts, and transportation for full coverage maintenance for G.E. Prestilex 1600, G.E. Advantix, G.E. MVP 80. Requires same day on site response, 30 minute field engineer (f.e.)response, next day parts delivery. Insite available, primary f.e., 2 preventive maintenance (pm) inspections per year, accessories discounts and 95% uptime guarantee. PM?s are due by 15 Feb 03 and 15 Aug 03. Limited services for G.E. MVP80 (15 hrs/year service calls). A government biomedical technician performs all PMs and ?first looks?. Hours of coverage 8-5 pm Mon-Fri. All parts (including glassware) to be covered under standard maintenance price. Detailed quote must specify billing frequency and all terms of coverage not specifically listed herein. Also your quote must list your DUNS number, CAGE code, and Federal TIN. If you need to obtain or renew a DUNS number or CAGE code, please visit www.ccr.gov. All contractors doing business with the federal government must be registered in CCR. DELIVERY ADDRESS: Services are to be performed for the 6th Medical Group (Bldg 711), 8415 Bayshore Blvd, MacDill AFB, FL 33621. PERIOD OF PERFORMANCE: 01 Oct 02 through 30 Sep 03. PROVISIONS/CLAUSES: The following FAR and DFARS provisions and clauses apply: 52.212-1, Instruction to Offerors-Commercial. In accordance with 52.212-2, Evaluation-Commercial Items, offers will be evaluated based on price, technical acceptability, delivery schedule, adherence to applicable clauses and provisions, as well as past performance records.
