NA This is a combined Synopsis/ Solicitation for commercial services and is prepared in accordance with the format specified in FAR Subpart 12.6, as supplemented with the additional information contained in this notice. Contact MAJ Kapec at 202-782-1103 if a copy of the solicitation is desired. Addendum/SOW CLINICAL PASTORAL EDUCATION TRAINER 1. GENERAL. 1.1. The Department of Ministry and Pastoral Care (DMPC), Walter Reed Army Medical Center (WRAMC), Washington, DC has a requirement for the part time non-personal services of a Clinical Pastoral Education (CPE) trainer. 1.2 A trainer accredited by the Association of Clinical Pastoral education (ACPE) is needed to provide direction, oversight, and training consistent with ACPE Standards and AMEDD 5G-CPE/SIT- 7R for the Supervisory Candidate (SC) Fellowship program, for t he period 1 October 2002 - 30 September 2003. 1.3. The trainer (contractor) shall manage and train the SC in accordance with ACPE Standards. This includes providing educational resources and conducting didactic training and peer group seminars. 1.4. Contractor is responsible for all CPE SC program activities. He/she will coordinate them with the Director of the CPE Site to facilitate and cooperate with WRAMC DMPC, and will coordinate program activities prior to their execution with the Chief, DMP C who has oversight responsibility. 2. CONTRACTOR QUALIFICATIONS 2.1. The contractor shall have completed a Bachelor?s degree and a Master of Divinity degree or equivalent in a field related to ministry. Contractor will have official transcripts verifying Bachelor Degree and Master of Divinity Degree forwarded by the a ppropriate institutions to Chief, DMPC no later than 7 days after award. 2.2. The contractor shall be a fully ordained clergy in good standing with his/her religious denomination and endorsed by his/her denomination to provide ministry as director of CPE. Contractor will provide to Chief, DMPC official certification of current ordination & good standing from his/her religious denomination dated not later than 2 weeks after accepting this contract. 2.3. The contractor shall have a minimum of three years active CPE supervision experience as a full-time ACPE accredited CPE supervisor. Contractor will provide to Chief, DMPC a certification statement from ACPE showing actual date of certification as CP E supervisor, and names and addresses of individuals who can verify employment history. 2.4. The contractor shall submit to a criminal history background check. A positive report is required for performance under this contract. Any criminal convictions or ethical/moral violations are grounds for disqualification or immediate termination of c ontract without further obligation incurred by the U. S. Government. 2.5. The contractor shall have established a proven track record of SCs supervised achieving Associate Supervisor status with ACPE. Provide to Chief, DMPC the name(s) of individual(s) supervised, with permission to contact them regarding their experience. 2.7. The contractor shall have ministry experience and training in trauma ministry. 3. CONTRACTOR PERFORMANCE REQUIREMENTS. 3.1. Contractor is responsible to the Chief, DMPC for provision of the Clinical Pastoral Education program SC program. 3.2. Contractor shall conduct and coordinate the CPE SC training program with Chief, DMPC. Coordination is to include: course changes, scheduling, work hour changes, emergency taskings, reports, invoices, performance documents, time keeping. In instances involving actions requiring higher headquarters? approval, or coordination, contractor will submit same to Chief, DMPC for further action. Also, all regional and national ACPE reports will be completed and submitted in time to meet the required deadlines. 3.3. Contractor shall direct, oversee, and guide the training of two SC (fellows). This also includes: evaluations, written and verbal, group and one-to-one interaction sessions, ensuring necessary committee appearances, assisting the SC in demonstrating reasonable progress toward full certification. 3.3.a. Contractor shall provide a letter of input to the Chief, DMPC when an Officer Evaluation Report is due for a SC. 3.3.b. Contractor shall provide unit evaluations to the SC in accordance with ACPE Standards and AMEDD Clinical Pastoral Education Courses. 3.3.c. Contractor shall accompany, if necessary, the SC for his appearance before any ACPE or AMEDD Center and School regional or national committees pertaining to associate certification. 3.4. Contractor shall meet the criteria and standards established by the Association of Clinical Pastoral Education in the most current edition of ACPE Standards and AMEDD CPE Courses, 5G-CPE/SIT- 7R. 3.5. Contractor shall coordinate each unit training schedule quarterly with the Director of the CPE Site or Chief, DMPC (or his representative) at least two weeks prior to beginning the unit. In the interim, should any changes become necessary, contractor shall coordinate the change in advance with Chief, DMPC or his representative. 3.6. The Contractor shall become current and remain current with all vaccinations in accordance with WRAMC Regulations 99-07 at no expense to the Government. 3.7. Contractor shall complete department statistical reports for documenting and tracking ministry and activities for the quarter. 3.8. Contractor shall complete finance paperwork for billing and payment. 3.9. Contractor shall provide his/her own ACPE certification fees to keep credentials and standing current, thereby remaining qualified for this contract. 3.10. Contractor shall provide transportation, per diem, and all other expenses to attend annual National ACPE conference and annual Regional ACPE conference as required. 3.11. Contractor shall present a professional appearance. While at WRAMC the contractor must wear the WRAMC identification badge. 3.12. Contractor may not subcontract any portion of this contract. Special presenters for didactics are not considered subcontracting. However, invitation of special presenters must be carried out under the guidelines of this contract. 3.13. Security Requirements. Contractor will comply with all WRAMC security requirements. This includes securing of office work areas, classroom, auditoriums, and equipment. Contractor will obtain a pass from the provost marshal for privately owned vehi cles and display that pass appropriately. 4. DEFINITIONS. 4.1. CONTRACTING OFFICER: Person with the authority to enter into, administers, terminate contracts and make related determinations end findings. 4.2. CONTRACTING OFFICER REPRESENTATIVE (COR): A Government employee selected and designated in writing by the Contracting Officer to act as his designated representative in administering a contract. 4.3. CONTRACTOR: A person or business firm offering services to the Government under this contract. 4.4. STAFF CHAPLAIN: The chaplain who is designated Deputy Chief, DMPC; Senior Chaplain Clinician; Religious Operations and Training Chaplain; and Chaplain Clinician are all designations in the department for the same chaplain. 4.5. QUARTER AND UNIT. In reference to CPE training periods one unit of training is the equivalent of one fiscal quarter or three months. 5. PERFORMANCE EVALUATION MEETINGS. 5.1. Contractor shall meet with the Chief, DMPC or his designee at least once per month. The contractor shall contact the WRAMC Chaplain?s office to make appointments for these meetings. 5.2. Purpose of these meetings is to discuss and evaluate the success of the CPE program by identifying CPE SC progress, significant CPE program issues, DMPC concerns, and any other issues pertaining to contract performance and CPE program success. The con tractor and/or the COR shall notify the Contracting officer of any discrepancies, disputes, or indication, of poor performance. Th e Chief, DMPC, if not the COR, shall notify the COR of such indications. 6. OTHER MEETINGS 6.1. Contractor shall attend other meetings as required by the Chief, DMPC for in-process review, coordination, planning of special events that involve CPE SCs, and other meetings or training required for hospital and department certification. 6.2. Contractor shall attend in-service training when deemed necessary by Chief, DMPC. Contractor will also assure that residents attend and support all required events according to the directives of the Chief, DMPC. 7. HOURS OF OPERATION. 7.1. Normal Hours of Operation. The contractor shall perform services required under this contract during the following hours: weekdays, 0730-1600. If CPE program needs indicate different days, these will be initiated only with the written approval of th e Chief, DMPC. 7.2. Holidays. Performance will not be required on the days listed in local clause ?Government Holidays?. 7.3. Absences shall be coordinated through the Chief, DMPC. 7.4. Consideration of emergency absences will be determined on a case-by-case basis at the discretion of the contracting officer with recommendation from the Chief, DMPC. 8. GOVERNMENT FURNISHED PROPERTY AND UTILITIES. 8.1. Office and equipment. The Government will furnish office space in the WRAMC CPE Center. Appropriate office furnishings and equipment for support of contractor in the actual performance of products and services will be provided. Provided office, fur nishings, and equipment are not permanent or dedicated solely for the use of the contractor, and are subject to availability. 8.2. Appropriate office furnishings and equipment includes: desk, desk chair, seating for student counseling and one-to-one interaction sessions, telephone, one filing cabinet, book case, and access to a computer. 8.3. WRAMC will provide identification badge to contractor. 9. CONTRACTOR FURNISHED PROPERTY AND UTILITIES. 9.1. Contractor shall provide for any fees or transportation necessary to reside in the Washington, DC area. 9.2. Contractor shall furnish his/her own transportation to and from the work site. 9.3. Contractor may use personal equipment, books, other literature, training aids and supplies to accomplish the requirements of this contract except as specifically noted and with approval of WRAMC DMPC Staff Chaplain. 9.4. The Government is not in any way liable for safekeeping, maintenance, upkeep, breakage, or anything else pertaining to use of personal property and equipment. 10. ADMINISTRATION. 10.1 Definition. Administration for the purpose of this contract is the completion of reports, Government forms, and other input that may be required for each contracted produce or service. 10.2. Documentation of Work/Reports. Contractor shall maintain a log of the work performed under this contract, and shall provide a copy of the log to the Staff Chaplain on a monthly basis. 10.3. Office hours are addressed in this contract under HOURS OF OPERATION. 10.4. Class location and schedules are addressed in this contract under CONTRACTOR PERFORMANCE REQUIREMENTS. 10.5. The Contractor will coordinate supply and equipment requests with the Director of the CPE Center. 10.6. The Director of the CPE Center will provide applicable forms, Standing Operating Procedures and instructions to the contractor. 10.7. Privacy Act. The contractor shall be familiar with and insure compliance with provisions of the Privacy Act, Freedom of Information Act, and limitation on Governmental collection of personal and statistical data on non-affiliated personnel. 10.8. Applicable Government regulations concerning these acts will be made available to the contractor for review by the WRAMC Chief, DMPC. 11. PHYSICAL SECURITY. 11.1. Contractor shall be responsible for safeguarding all Government property provided for contractor use. At the end of each work period all Governm ent facilities, equipment and materials shall be secured. 11.2. To prevent unauthorized access to Government software and the Internet, the contractor shall log off when away from any Government computer used. 11.3. Key Control. Contractor shall assure that all keys issued to him/her by the Government are safeguarded against lose, theft, and use by unauthorized persons. This includes use and safekeeping of the identification badge, which permits entry via elec tronic readers. The contractor shall not duplicate any keys issued by the Government. 11.4. Breach of this requirement may require contractor to replace keys, rekey, and replace locks without cost to the Government. 12. RECORDS. 12.1. Contractor shall be responsible for creating, maintaining, and securing required SC records as defined by ACPE standards. If requested by the Government, the contractor shall provide the original record, or a reproducible copy of the record within 5 working days of receipt of request. 12.2. Contractor will coordinate with Director of the CPE Center to ensure records of certifications, fees, training credits, and other required documentation in accordance with current ACPE Standards and military requirements. 13.
