- Description
- This is a combined synopsis/solicitation for commercial items prepare= d in accordance with the format in FAR, Subpart=20 12.12 as supplemented with additional information included in this notice. This notice constitutes the only solicitation;=20 offers are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request=20 for Quotation (RFQ) number 688-23-03. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular number 97-9. The NAIC code for this solicitation is 811310. The small business size standard is $5 million. This solicitation is not a small business set-aside. This service=20 contract shall include a multi-year request for support to cover the General Electric Medical System LC+ Vascular and the Innova 2000 Cardiac Cath Labs as well as the imaging workstations and system storage device located in Cardiology Service at the VAMC Washington, DC. Also included in the contract is the GE 1.5T CVI MRI located in Imaging Service. The Vendor will provide all labor, tools, travel, equipment, and parts required to perform repair (including glassware)and two (2) preventive maintenance inspections(PM) on equipment covered by the contract at the VA Medical Center at 50 Irving Street, NW, Washington, DC 20422 for the period of 10/1/02 thru 9/30/03 in accordance with all terms, conditions, provisions, schedules, and specifications of this solicitation. All equipment shall be maintained in proper operating condition per the model specific manufacturer=92s service manual. Only original equipment manufacturers (OEM) parts, manuals, and schematics will be utilized to perform the service. These must be available on site at time of service. All defective parts replaced become the property of the vendor. Normal working hours are Monday through (8:00am until 4:30pm)=20 and will cover the period October 1, 2002 through September 30, 2003 , with two (2) , 1-year options, in accordance with all terms and conditions, provisions, schedules and specifications of this solicitation. Award of this contract is subject to the availability of funds and may be extended for two (2) additional option years in accordance with the clause at 52.217-9, Option to Extend the Terms of the Contract.
