- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number FA4416-00-T0406 is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 97-17. This is a 100% small business set aside. The SIC code is 2879. The size standard is 500. The contractor shall provide all management, tools, supplies, equipment, and labor necessary to ensure that pest control services are performed at Andrews AFB MD in accordance with the DRAFT Description of Work. The provision at FAR 52.212-1, " Instructions to Offerors-Commercial Items and addenda's. FAR 52.237.1 Site Visit and FAR 52.228-5 Insurance - Work on a Government Installation, is applicable to this acquisition. The provision FAR 52-212-2 Evaluation--Commercial Items is applicable to the solicitation and the blanks in (a) are completed as follows: Price and Past Performance. Offerors shall include with their offers, a completed copy of 52.212-3, Offeror Representations and Certifications--Commercial Items; FAR clause 52.212-4, Contract Terms and Conditions Commercial Items is applicable to this solicitation. FAR clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items is applicable and the following clauses are checked: 52.232.33, Payment by Electronic Funds Transfer-Central Contractor Registration and 52.222-41 Service Contract Act of 1965, as amended (41 U.S.C. 351, et seq.). DFARs clause 252.204-7004 Required Central Contractor Registration is applicable to the solicitation. Attendees shall meet at Bldg 1840, Main Gate Visitor Center, Andrews Air Force Base MD and should contact the contract specialist below by 22 June 2000 before 3:30 PM Eastern Standard Time to be granted access to the base. Responses to this RFQ must be received via fax, mail or delivery by 28 June 2000 no later than 2:00 PM Eastern Standard Time at the 89th Contracting Squadron, 1419 Menoher Drive, Andrews AFB Maryland. They should be marked with solicitation number and addressed to Jim Harris. Vendors not registered in the Central Contractor Registration (CCR) database prior to award will be ineligible for award. Vendors may register for CCR by calling 1-800-334-3414 or register on line at www.CCR2000.com. Contractors are requested to review the Description of Services DRAFT on EPS (Electronic Posting System). Contractors are required to check the following internet site regularly for amendments: www.eps.gov, USAF- Air Mobility Command- Andrews.
