- This is a combined synopsis/solicitation in accordance with the format in FAR Subpart 12.6, and FAR 13.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number, USZA99-03-Q-0041 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-12. This procurement is restricted to small business set aside. All responses from responsible sources will be fully considered. Clin 0001, Tube Restraint easily and firmly secures endotracheal tubes under field conditions. 100 ea.; Clin 0002, Duraskock BP Cuff, durable, meet the AAMI SP9 specifications for shock resistance, bladderless, latex-free cuff, size guide marking system, color black, 30 ea.; Clin 0003, Rusch Flexi-Set 8.0, a combination set of a cuffed endotracheal tube supplied together with a flexi-slip stylette. High volume/low pressure cuff is sized proportionately to tube size. Black positioning ring visually assists in prober placement below the larynx. Pilot ballon allows for tactile sense of cuff volume. Depth markings with full length x-ray opaque line. Flexi-slip stylette is plastic coated for easy withdrawal with a malleable metal core, 150 ea.; Clin 0004, Rusch Flexiset 7.0, a combination set of a cuffed endotracheal tube supplied together with a flexi-slip stylette. Solicitation Provision at FAR 52.212-1, Instructions to Offerors Commercial Items (Oct 2000) is hereby incorporated by reference. Solicitation Provision FAR 52.212-2, Evaluation Commercial Items (Jan 1999) is hereby incorporated, the evaluation factors are technical, price, and past performance. The government will make a determination of best value based on the evaluation factors cited. Contractors are reminded to submit a complete copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items with their quote, if readily available.
