- NA This is a combined synopsis/solicitation for commercial items/service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; prop osal are being requested and a written solicitation will not be issued. This solicitation DABJ25-03-R-0021 is being issued as a Request for Proposal. This solicitation document and incorporated clauses/provisions are those in effect through Federal Acquisi tion Circular 2001-013. NAICS Code is 611420 and the size standard is $5 mil. This solicitation is issued as full and open competition. The ACA Fort Lewis intends to award a firm fixed price contract for the following commercial item(s). CLIN 0001: On-l ine Technology Pilot Program on-site Instructors/Monitors. Two Instructors are to monitor the computer center(s); CLIN 0002: On-line Technology Pilot Program over and above hours for instructors/Monitors of the computer center(s) incorporated within the PW S; CLIN 0003: On-line Technology Pilot Program, computer course subscriptions; CLIN 0004: On-line Technology Pilot Program, Youth Cyber-Computer Subscriptions. Ship to address is DOL Central Receiving Point ATTN: Billy Goforth, Bldg 9630, Fort Lewis, WA 98433-9500. Request FOB Destination pricing. Basis of Selection will be technical capability, price, and past performance. Offerors will submit along with their pricing the following information for evaluation. A discussion of the company??????s abilit y to obtain or provide the service required under this request for proposal in accordance with the performance work statement. Offerors shall also provide a maximum of two references with whom recent past performance can be verified. Provide the scope of work, the contract number, dollar value, name, title, and a telephone number of the person with whom to speak regarding your performance related to conformance with contract requirements; standards of good workmanship; adherence to contract schedule; reaso nable and cooperative behavior and; commitment to customer satisfaction. Offerors are responsible for accurate addresses and telephone numbers for submitted past performance information. The Government as part of the evaluation process may consider informa tion that is obtained from other sources. NOTE: Proposals submitted which do not include a written technical representation that addresses the evaluation factors may not be considered for selection. Technical capability is slightly more important than pr ice or past performance, which are equal. The Government reserves the right to select a offer other than the lowest price. The following FAR clauses and provisions apply to this solicitation: FAR 52.212-1 Instructions to Offerors-Commercial Items; FAR 52 .212-3 Offeror Representations and Certifications-Commercial Items; DFARS 252.212-7000, Offeror Representations and Certifications Commercial Items (all offerors are to include with their response a completed copy of FAR 52.212-3 and DFARS 252.212-7000). Further, offers must contain all commercial terms, conditions and provisions under which the offeror conducts business in the commercial marketplace, and warranty terms and conditions. Offers that fail to furnish required representations or information, o r reject the terms and conditions of this solicitation may be excluded from consideration.
