Description
This is a combined synopsis/solicitation for commercial items or services prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. This solicitation is a Request for Quote (RFQ) No. V230203. These solicitation clauses and provisions are those in effect through Federal Acquisition Circular 97-27. The Standard Industrial Classification (SIC) Code is 7389 and the North American Classification System (NAICS) Code is 541990. The Center for Veterinary Medicine (CVM) is looking to develop a comprehensive computerized inventory of compounding pharmacies. This project will be completed in three phases. The first phase will focus on building the database and incorporating appropriate data fields. The second phase will consist of identifying firms to be included in the inventory. The third phase entails gathering requested data and entering it into the database. Phase I: Develop a database, ideally using ACCESS, which can be operated on a PC with Windows 2000 Operating and LAN systems. The database will be dynamic so that it can be updated. Phase II: Using a variety of sources including, but not limited to, State Boards of Pharmacies, FDA District Offices, Veterinary Journals, Veterinarian Interviews, Trade Magazines, and Internet searches develop a comprehensive list of compounding pharmacies. Phase III: The inventory should include the firm name, full address, telephone number, website, and approximate size of business. Please include same information for all affiliated firms. Include the State(s) of licensure, and whether the firm compounds only veterinary drugs or also human drugs. Include the names of all drugs being compounded and the sources of these drugs. Include the species/slaughter class of animals in which the drugs are to be used. Also, provide CVM with a hardcopy file of all promotional materials and price lists catalogued by firm. Reporting Requirements and Deliverable Items: The deliverable is a comprehensive database with all requested data collected and entered. The supporting documentation will be provided and catalogued by firm name. The supporting documentation will include all sources for the data and what, if any, information was obtained from those sources. The contractor will visit CVM prior to starting the project to discuss the objectives and deliverables. There will be quarterly meetings with CVM during the contract period to report on progress and conduct a demonstration of the database and data collected. The contractor will also submit quarterly reports and a final report (the fourth quarter report) to the project officer. The reports will contain all accomplishments since the previous report and the activities planned for the next quarter. The reports will contain a functional copy of the database. Duration of Project: This project is anticipated to be a 12-month contract. Evaluation Criteria for Competitive Proposals: (A) Qualifications of Personnel: (35%) Offerors shall demonstrate that they have the personnel requisite to perform high caliber surveillance data collection. A background in veterinary medicine and/or pharmacy would be extremely useful. Offerors shall indicate which personnel would be participating in individual tasks and what percentage of their time would be devoted to this task. (B) Organizational Experience: (25%) Database Development: Offeror shall demonstrate that they have the personnel requisite to develop a well-designed database to organize, maintain, and present the information obtained on compounding pharmacies. If a subcontractor (developer) is used, the qualifications of the subcontractor for this task must be sufficient to provide a high-quality product. ( C ) Technical Approach: (25%) Offeror shall indicate how they would approach tasks stated in scope of work involving elements of database development and data gathering. The offeror should also demonstrate their ability to obtain information from a variety of sources. The offeror shall indicate familiarity with Government data sources as well as primary data collection capabilities. The Contractor shall describe facilities and computer capabilities, and ability to maintain frequent liaison with the Food and Drug Administration (CVM) on this project. Access to computer services shall be crucial for certain quick response tasks. (D) Quality Control Procedures: (15%) The Contractor shall identify means of final draft quality to ensure that it is relatively free of typographical errors and any other quality control steps should be taken as necessary. The Government expects the contractor to carefully check data submitted and report accurately as to any potential method of compliance with this issue. Site Visit Schedule: The Project Officer will visit the contractor site within the first six months of the contract. The following FAR clauses shall apply to this solicitation: 52.212-1 Instructions to Offerors – Commercial; 52.212-4 Contract Terms and Conditions-Commercial Items; and 52.232-33 Electronic Funds Transfer. All responsible sources that can provide and meet the above requirements shall submit written quotations by the due date referenced above. Evaluation/award will be based on the technically acceptable quote that offers the best value to the government. All quotes shall include price (s); a poc (name and telephone number); GSA contract number if applicable; business size, and payment terms.
