Description
The Department of Veterans Affairs (VA) Medical Center, San Juan, Puerto Rico intends to negotiate a commercial item (FAR Part 12) Brand Name or Equal contract for labor, installation and supplies of seven (7) Horton Automatic Sliding Doors including components and one (1) overhead swing electric operator (Lockout relay module). Door shall have breakaway options that permits full opening of the sliding panels; fixed panels shall act as swinging doors in the event of power failures. This is a combined synopsis/solicitation for Commercial Items prepared in accordance with the format in the Federal Acquisition Regulation (FAR), Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. Solicitation Number RFQ672-296-03 is issued as a request for quotation (RFQ). The incorporated document, provisions and clauses are those in effect through the Federal Acquisition Circular 2001-15 effective August 25, 2003 except for items III, VII & VIII which are effective July 24, 2003 and are incorporated into this RFQ by reference (FAR) clause website is www.arnet.gov/far/ http://www.arnet.gov/far/. FAR 52.212-1 Instructions to Offerors-Commercial Items (July 2003), 52.212-3 Offeror Representations and Certifications-Commercial Items (June 2003), 52.212-4, Contract Terms and Conditions-Commercial Items (Feb 2002), FAR 52.212-5Contract Terms and Conditions-Commercial Items (June 2003), Contract Terms and Conditions required to Implement Status or Executive Orders-Commercial Items. Applicable Clauses in FAR 52.212-5 paragraph (a) are 52.222-3, 52.233-3 and in paragraph (b) are: 52.222-21,52.222-26, 52.222-35, 52.22-36, 52.222-37, 52.222-19. 52.225-3, 52.225-13, 52.225-15, 52.225-16,52.232-33,52.232-34, 52.247-64. The provision at 52.212-2 Evaluation-Commercial Items (Jan 1999) does not apply to this acquisition. Instead, the following information will be used for evaluation of offerors: an award shall be made to that responsible offeror submitting a technically acceptable quotation and offering the lowest evaluated price. Price shall be the deciding factor among technically acceptable quotation. To be technically acceptable, the offeror=92s technical quotation or proposal must be evaluated as technically acceptable for each of the requirement listed above. Technical acceptability will be determined solely on the content and merit of the information submitted in response to this provision.=20 Therefore, it is incumbent on the offeror to provide sufficient technical literature, documentation, item specification sheets or brochures, etc., which will describe the =93Equal=94 for the Service evaluation and decision. The price proposal shall be evaluated on the overall lowest price to the Government. This action is 100% set aside for small business. The applicable NAIC is 235950. The doors and components must be compatible with =93Horton=94 specifications. Therefore, the contractor shall meet all of the specific requirements. CLIN 001- Three (3) Automatic Sliding Door- Bi-Parting Slide Door; Slide Opening (72=94); Clear or Bronze Anodized finish will be determined by Service; Glass: =BC=94 Clear Safety; Opening: 6=92-0=94 x 7=92-0=94=94; Door Size: 1= 4=92-0=94 x 7=92-0=94; Telescoping with side light. CLIN 002- One (1) Automatic Sliding Door-Bi-Parting Slide Door; Slide Opening (72=94); Clear or Bronze Anodized finish to be determined by Service; Glass: =BC=94 Clear Safety; Opening: 5=92-0=94 x 7=92-0=94; Door Size: 12=92-0=94 x 7=92-0=94; Telescop= ing with side light. CLIN 003- One (1) Automatic Sliding Door with side-light; Header-extended; Clear or Bronze Anodized finish (will be determined by Service); Glass: =BC=94 Clear Safety; Opening: 6=92-0=94 x 7=92-0=94; Door = Size: 9=92-0=94 x 7=92-0=94; Single Slide Door Telescoping-Max. Slide 5=92-5=94(6= 7=94). CLIN 004-One (1) Automatic Narrow Sliding Door with Mounting Bars Dual Safety Beams; Two (2) Push Plates; Clear or Bronze Anodize finish (will be determined by Service); Glass: =BC=94 Clear Safety; Opening: 7=92-6=94 x 7=92-10=94; Door Size: 7=92-6=94 x 7=92-4=94. CLIN005- One (1) Automatic Si= ngle Telescoping Sliding Door; Door: Narrow Stile with 6 =BD=94 Bottom Rail; Glass: =BC=94 Safety Clear; Optional Mid Panel: Horizontal Mounting; Lock Indicator; Two (2) Motion Detectors Dual Safety Beam; Opening: 9=92-0=94 x 7=92-2 =BD=94 / 7=92-6=94; CLIN 006- One (1) Overhead Swing Electric Operat= or with Lockout Relay Module to be installed in existing Double Door with Motion Present Sensor, all wiring; Opening: 6=92-4=94 Long Double Door. Total Price$_________. Delivery Terms: FOB Destination (Door to Door).=20 The Contractor shall deliver Doors within 30 days or less after contract is awarded to Department of Veterans Affairs VA Medical Center, Warehouse loading Dock Area; 10 Calle Casia , San Juan, Puerto Rico 00921-3201. Inspection and acceptance shall be made at destination. Warranty-The Contractor shall extend to the government full coverage of any standard commercial warranty normally offered in a similar commercial sale. Acceptance of the warranty does not waive the Government=92s rights under the Inspection clause, nor does it limit the Government=92s rights with regard to the other terms and conditions of this contract. In the event of a conflict the terms and conditions of the contract shall take precedence over the warranty. The Government contemplates award of a Firm Fixed-Price contract resulting from this solicitation.
