- Description
- Contractor shall furnish artificial limbs and related services to those veterans that require them and are eligible for assistance at The Louis A. Johnson VAMC, One Medical Center Drive, Clarksburg, WV 26301. The Contractor shall fabricate all artificial limbs in strict conformance to the prosthetic prescription that has been provided by the VA Amputee Clinic Team. Contractor shall not add or substitute components or alter the beneficiaries limb prescription in any way without prior approval and written authority from the VA Amputee Clinic Team Prosthetic Representative or designee. All offers must submit technical and price proposals. The technical evaluation criteria for this procurement include education, facility, management approach, and past performance. The pricing will be based on percentage discounts against L-Code pricing baseline. The Government anticipates issuing multiple awards from this solicitation. The VA will award no more than four (4) fixed price indefinite quantity contracts pursuant to this solicitation. This acquisition is a total 100% small business set-aside. All vendors submitting proposals in response to this solicitation shall have a service center physically located within a thirty (30) mile radius of the VA Medical Center in Clarksburg, WV. VA shall determine (based on an integrated assessment of each Offeror?s technical and price proposals) the individual proposals judged to be most advantageous to the Government. Contract period will begin o/a November 1, 2003 through October 31, 2004, with two (2) one (1) year renewal options all subject to the availability of funds. Electronic solicitations will be available on the internet o/a 9/26/2003. Interested parties are required to submit prices and clear and convincing data substantiating their qualifications to satisfy this requirement. Proposals are due by 2:00 p.m. on October 24, 2003 and should be mailed to the Contracting Officer (542/90C), VAMC, 1400 Black Horse Hill Road, Coatesville, PA 19320. Electronic offers will not be accepted. See numbered note 1.
