Description

NA Combined Synopsis/Solicitation for Panoramic x-ray machine and peripherals. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation DAHA41-03-T-0091 is issued as a Request For Quotation (RFQ) which incorporates provisions and clauses i n effect through Federal Acquisition Circular 14, effective as of 6/23/03. This is a total small business set-aside acquisition with an associated NAICS of 423450, small business size standard of 500 employees. The Small Business Competitiveness Demonstr ation Program is not applicable. Description of Panoramic x-ray machine and peripherals is as follows: Item 1 is for Three (3) each Soredex Cranex Excel D or equal Panoramic x-ray machine, must be digital ready, Pano must have a FOCAL SPOT size no larger than 0,5mm, must have a 4-point head support system, must not exceed 38 inches in width and not to exceed 47 inches in depth, must have the Auto set KV and MA features for automatic sizing of the patient, and must have local and statewide service support available from an authorized service company for the Sordex or equal Panographic equipment; Item 2 is for Three (3) Schick Maxi-User CCD or equal software for digital processing and printing and must be FACTORY INSTALLED into and compatible with the Panoramic x-ray machine; Item 3 is for Three (3) Mounts for monit ors and keyboards, Ergotron P/N 27-404-100 or equal, wall mountable; Item 4 is for Three (3) H/P Photo-Smart printers minimum 7530 series, 17PPMB & W or equal with black ink cartridge and compatible with the computer; Item 5 is for Three (3) Computers, Del l Dimension 8300 Series or equal, pentilum IV, 2.8 ghz processor, 256mb RAM, Logitech Keyboard/Optical Mouse, 17-inch Dell or equal Flat Panel Monitor, 80 gb hard drive, floppy, Windows XP Pro, Intel Pro NIC, 48x CD RW, WordPerfect software, McAfee Anti-Vi rus, 3-Year on-site service, 80 pk CD-R, battery backup 350. Must be compatible with the digital panoramic x-ray machine; Item 6 is for Three (3) Installation for computers. If other than brand names for Items 1 through 6 is offered, the submission of do cuments to substantiate the salient characteristics is required with quote. DELIVERY ADDRESSES are as follows: One (1) each of Items 1 through 6 goes to the Texas Medical Command, Attn: SSG Polozeck, 1013 Lake Crest Dr., Grand Prairie, TX 75051-3496; O ne (1) each of Items 1 through 6 goes to the Texas Medical Command, Attn: SSG Toussant, Bldg 1182, Ellington Field, Houston, TX 77034-5596; and the remaining One (1) each of Items 1 through 6 goes to the Texas Medical Command, Attn: SGT Jacquez, 4255 IH 35 N, Amory # 1, San Antonio, TX 78219-1600. The delivery date will be coordinated with the vendor at the time of awarding the contract.
