Description
This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-03-T-0112 is issued as a request for quotation (RFQ). Clin 0001. Contract for Daytime priest Support for DPC Service required: Non-personal contract for a Roman Catholic Priest, IAW Statement of work (SOW). Period covered will be 01 OCT 03 through 30 SEP 04 (base year); Clin0002.Option year 01 Oct 04 - 30 Sep 05 (one year option) FAC 2001-15. Prior to Contract Award, the government reserves the right to review the resumes of contractor employees performing under the contract solely for the purpose of ascertaining their qualifications relative to the personnel qualificationsterms of the contract. All quotation must be received by September 23, 2003, @ 2:30 Pacific Standard time to be considered responsive. STATEMENT OF WORK NON-PERSONAL ROMAN CATHOLIC PRIEST SERVICES,I. INTRODUCTION/SCOPE Background. The commander Naval Medical Center San Diego (NMCSD) is responsible for providing the spiritual and religious care of patients and staff and supporting the Naval Bureau of Medicine and Surgery (BUMED) mobilization requirements. BUMED provides chaplain billets to support these requirements. The Chaplain Corps provides chaplains to fill these billets. The diverse variety of religious denominations among patients and staff require a proportionate denominational mix of chaplains. Discussion. The Roman Catholic faith group is a significant cross section of patients and staff requiring religious support at NMCSD. Approximately 30% of the hospital nature of pastoral care to Roman Catholics is sacramental in nature. The celebration and administration of sacraments requires the presence of a Catholic priest. Therefore, it is essential that Catholic priests are available to provide pastoral care and sacraments to Roman Catholic patients and staff at NMCSD and support deployments. Traditionally, the Navy Chaplain Corps has been able to provide and adequate number of Catholic priests to fill billet requirements and care for the Roman Catholic population within the naval sea services. In recent years, a shortage STATEMENT OF WORK NON-PERSONAL ROMAN CATHOLIC PRIEST SERVICES,I. INTRODUCTION/SCOPE. Discussion. The Roman Catholic faith group is a significant cross section of patients and staff requiring religious support at NMCSD. Approximately 30% of the hospital nature of pastoral care to Roman Catholics is sacramental in nature. The celebration and administration of sacraments requires the presence of a Catholic priest. Therefore, it is essential that Catholic priests are available to provide pastoral care and sacraments to Roman Catholic patients and staff at NMCSD and support deployments. of Catholic priests in the civilian community has developed. This shortage is now reflected in the decreased availability of Catholic chaplains in the naval sea services everywhere. The decreased availability of Catholic Chaplains has created a situation in which adequate Catholic coverage is not available at NMCSD by Catholic chaplains are not available. Therefore, we need to diverse and implement new strategies to meet the spiritual and religious needs of Roman Catholics. This requires us to use Roman Catholic assets as efficiently as possible. There is also a need to identify new and additional sources for Catholic sacramental ministry. Scope. As a result of the above issues, requirement exists to contract for the non-personal services of a Roman Catholic priest to assist the Navy Catholic Chaplain to provide for the pastoral and sacramental needs of Roman Catholics at NMCSD. The civilian priest would be an independent contractor of the Pastoral Care Service chaplain team, administering sacraments, offering spiritual care and counsel, celebrating daily Mass, and responding to the other Catholic religious needs as they may arise. 2. CONTRATOR REQUIREMENTS. The contractor shall provide sacramental and pastoral care and counseling to staff, patients and their families, prepare and maintain records, be available to celebrate daily Mass, participate in meetings required by the Director of Pastoral Care service, and respond to other Catholics needs as they may arise. During the term of this contract, the contractor (civilian priest) agrees to perform on behalf of the U.S. Government, the duties of a Roman Catholic priest for the spiritual care of active duty military personnel, their dependents, eligible Navy civilian employees, and other beneficiaries, per the terms and conditions of the contract. As an independent contractor, all medical, dental, and professional liability insurance is the responsibility of the contractor. 3. REFERENCES: The following documents and data sources are expected to be needed by the contractor in assisting the Navy Catholic Chaplain to provide pastoral and sacramental care, and will be provided upon request. This information will be provided by the medical center?s Pastoral Care Service staff: A. Demographic information on the beneficiary population served. Current data on patient preferences as determined by survey, correspondence, and patient relations statistics. Information regarding non-medical issues such as facilities, parking, support services, staffing, watch bills, and other amenities. Results of inspection/surveys by internal and external agencies. Assessment of employee moral and job satisfaction as determined by annual survey of entire staff. POPERTY. Contractor-Furnished Property. The contractor shall finish any computer hardware and software necessary for use by the contractor. Government-Furnished Property. The government will provide office space, office furniture, telephone/fax service, and the use of a copier. The government will allow unlimited telephone usage, both local and long distance, in support of the Pastoral Care Service program. The government will provide the referenced information specified in Section 2. The Pastoral Care Service staff will coordinate the retrieval of information needed by the contractor and serve as the interface. The government will provide resources for the administration and celebration of sacraments and other professional resources deemed necessary by the Director of Pastoral Care Services. The government will provide office staff for administrative support. 5. OTHER CONSIDERATIONS/REQUIREMENTS: Place of Performance. Work shall be performed on site. Meetings between the contractor and the Director of Pastoral Care, or his representative, shall be performed on site. Work Hours. The contractor shall work twenty-eight hours (28) per week. Monday through Friday, fifty-two (52) weeks per year. Performance shall normally be during the hours of 0700-1500. This extended workday shall follow the duty of the Navy Catholic Chaplain. Flex hours may be employed at the mutual consent of the contractor and the Director for Pastoral Service. The contractor shall also be responsible for six Sunday Masses during the course of the year. The dates of the Masses are to be mutually agreed upon with the Navy Catholic Chaplain. Four hours of credit will be given to each Sunday Mass celebrated. Leave and holidays will be approved by the Director Pastoral Care Service in accordance with NMCSD policy. Communication. The contractor is authorized to meet with Department Heads, Directors, supervisors, and other staff personnel as well as patients and their families in the performance of pastoral responsibilities. The contractor shall pursue all administrative matters through the Director Pastoral Care Service. The Director of Pastoral Care Service will provide a written Performance evaluation to the contractor mid-yea and at the close of the fiscal year. Personal Qualifications. The contractor shall staff the contract with a validity ordained Roman Catholic priest in good standing with his diocese or religious order and endorsed by the Archdiocese for Military Services. The Ecclesiastical Endorsements shall be submitted in writing. The catholic priest shall demonstrate reliability, good relational skills and the ability to serve on an ecumenical ministry team. The contracted priest shall comply with government policies and regulations in regard to identification badges, parking, and professional decorum. Primary Point of Contact. The senior Navy Catholic Chaplain (Roman Catholic Program Coordinator) on staff at NMCSD is the primary point of contact for the contracted priest. No alien shall be allowed to perform under this contract without an Alien Registration Card (?green card?) and/or in violation of the Immigration Laws of the United States. The contractor shall be neat, clean, well groomed, and in appropriate clergy clothing (shirt with Roman collar) when in patient care and public areas. The contractor shall display an identification badge on the right breast of his/her clothing, which includes the contractor?s full name, professional status, and expiration date. The contractor shall become acquainted with and obey all pertinent Command, Navy, and DOD regulations, shall perform in a manner which precludes any waste of utilities and shall not use Government telephones (of other Government equipment, e.g. loaner pagers) for personal business. All motor vehicles operated on this installation by the contractor shall be registered with base security and operated according to applicable directives. Eating and smoking by the contractor are prohibited in patient care areas and restricted to designated areas. The contractor shall be able to read, write, speak, and understand the English language fluently. Unplanned absences due to illness or other incapacitation of the contractor will be allowed up to a maximum of 15 days per contract year. The contractor shall advise the COR (senior Navy Catholic Chaplain, the Roman Catholic Program Coordinator) of such absences. If the contractor is absent for 4 or more weeks due to illness, he shall provide a written documentation from a qualified health care provider that he is free from any communicable disease. The government reserves the right to examine or reexamine any such contractor who meets this criterion. The contractor shall provide a competent priest substitute, with the approval of the Director of Pastoral Care Service and the senior Navy Catholic Chaplain, will do Catholic coverage during periods of contractor absences (not to exceed 30 days per year). A physical examination certification, which states the date on which, states the date on which the physical examination was conducted, the name of the doctor who performed the examination, and a statement concerning the physical health of the contractor. The certification must contain the following statement: ?(Name of Contractor) is suffering from no physical disability or medical condition which would restrict or preclude his/her from providing services as a Roman Catholic Priest. (Name of Contractor) is suffering from no contagious diseases to include but not limited to AIDS, tuberculosis, hepatitis, and venereal disease, and is not positive for HIV antibodies.? A resume must be submitted in advance for technical review including total annual salary based upon hours to be worked under contract. If this information is proprietary to subcontractors, it may be provided under separated cover; however, it must be easily identifiable and readily combined with the rest of the proposal. Failure to provide this information may impact the Government?s evaluation of the contractor?s proposal. INTERVIEW. The Senior Roman Catholic Chaplain conjointly with at least one other NMCSD staff chaplain will make and interview of each applicant. The interview will reflect a review of the overall package with emphasis on the following: Overall professional training and experience of the candidate as a hospital chaplain. Cost of the contract; Military experience and/or understanding of military methods by the candidate. Flexibility of the candidate in meeting the needs of the hospital patients and staff within the parameters of the contract. Perceived ability of the candidate to do sustained and intense hospital ministry. Perception of the candidate as a team player who can edify and be edified by the Pastoral Care Directorate team. Overall evaluation of package and all of the above. Cost of the contract,Military experience and/or understanding of military methods by the candidate.Flexibility of the candidate in meeting the needs of the hospital patients and staff within the parameters of the contract. Perceived ability of the candidate to do sustained and intense hospital ministry.Perception of the candidate as a team player who can edify and be edified by the Pastoral Care Directorate team.Overall evaluation of package and all of the above.
