NA PERFORMANCE WORK STATEMENT (PWS) 1.0 SCOPE OF WORK 1.1 This contract covers the delivery and pick-up points of documents between the 833d U.S. Army Trans. Bn., and those locations specified below. 1.2 Pick-Up/Delivery Points 1.2.1 833d Trans Bn 4735 E. Marginal Way South Seattle, WA 1.2.2 American Fast Freight 901 Alexander Avenue Tacoma, WA 1.2.3 Alaska Marine Lines 5615 W. Marginal Way South Seattle, WA 1.2.4 Unified Aircraft Services 2302 Ross Way Tacoma, WA 1.2.5 Blair Terminal 2421 Port of Tacoma Road Tacoma, WA 1.2.6 Boyer Alaska Barge Lines 6700 W. Marginal Way South Seattle, WA 1.2.7 833d Trans Bn 4735 E. Marginal Way South 1.3 The Contractor shall pick-up documents and other distribution items at the 833d Trans Bn; 4735 E. Marginal Way S.; Seattle, WA., after each federal workday; deliver envelopes to appropriate points; pick-up documents and other distribution items at the same points and return them to the 833d Trans Bn. At the above address prior to 7 AM the following federal workday. All deliveries must be completed between federal workdays; that is, no documents or other distribution items will be in the possession of t he Contractor during normal working hours of the 833d Trans Bn.. The Contractor will not provide service on any federally observed holidays or weekends. 1.4 Listed below are the ten federally observed holidays for which no service will be required: Columbus Day Veterans Day Thanksgiving Day Christmas New Years?????? Day Martin Luther King??????s Birthday President??????s Day Memorial Day Independence Day Labor Day 1.5 The Contractor shall provide and install lock boxes at their own expense at all locations specified in Section (1.2), Pick-up and Delivery Points. The measurements are to be approximately 28?????? long x 14?????? high x 10?????? wide and positions at the height from ground level that is reasonable for the location. 1.5.1 There may be a need to add or drop a pick-up/delivery point or change locations of a pick-up/delivery point. Should this occur the Contracting Officer and the Contractor will address the need to negotiate a price change and/or change to the scope of work to the contract. These proceedings will be guided by FAR Clause 52.243, Changes, Alt I (Aug. 1987) 2.0 PERFORMANCE/DELIVERIES 2.1 Performance on this acquisition shall be required from October 1, 2003 through September 30, 2004. 2.2 Performance as described herein will be monitored by the Contracting Officer. If any actions by the contractor are perceived to be non-compliant with the contract terms the Contracting Officer will request a statement from the Contractor of the cause for the non-compliance and a corrective action plan to prevent future occurrences of the non-compliance 2.2.1 Any loss of documents, files or envelopes will be considered a major non-compliance and could result in a termination of the contract for default which proceedings are described in FAR Clause 52.249-8, Default. 2.2.2 Damage to documents, files or envelopes caused by the Contractor coul be a major or minor non-compliance and will be evaluated on a case-by-case basis. 2.2.3 Persistent problems in contract performance, whether minor or major, could be cause for termination of the contract and will be used in the Contracting Officer??????s past performance report at contract conclusion. 2.2.4 Disputes arising from performance issues on this contract will be guided by FAR Clause 52.233-1. 3.0 SECURITY 3.1 The Contractor shall be responsible for all documents and other distribution items within its control (i.e. from the point of pick-up to the point of delivery). The Contractor shall be relieved of responsibility when the documents and other items of d istribution are securely locked in the lock-box at each pick-up/delivery point. 4.0 SUPPLIES AND EQUIPMENT FURNISHED BY THE GOVERNMENT 4.1 Distribution envelopes 4.2 Lock-Box at the location of the 833d Trans. Bn. Offi ce only. 5.0 SUPPLIES AND EQUIPMENT FURNISHED BY THE CONTRACTOR (at own expense) 5.1 Vehicles required to perform pick-ups and deliveries 5.2 Lock boxes at the locations specified in paragraph (1.2), Pick-up/Delivery Points of the PWS, with the exception of that which is located at the 833d Trans. Bn. 6.0 PAYMENT/INVOICES 6.1 The Contractor shall be paid in accordance with the FAR Prompt Payments Clause, 52.232-25. 6.2 Invoices shall be submitted by the Contractor on a monthly basis, after the services have been provided. Invoices shall be submitted in an original and three copies to the Contract Administration Office (ACO) identified in Block (6.) of DD Form 1155, Order for Supplies and Services.
