Description
Solicitation W912QR-04-R-0008, under Description, delete in its entirety No. 3. SELECTION CRITERIA and replace with No. 3 SELECTION CRITERIA below, and also add ORGANIZATIONAL APPROACH 3. SELECTION CRITERIA: For Pre-Solicitation review, Contractors are required to submit a brief statement of qualifications (SOQ). The format of the SOQ should address the following areas Project Experience and Organizational Approach: PROJECT EXPERIENCE ; Create and complete a table (Word or Excel) or a narrative consisting of the following information for a maximum of 10 projects (i.e. contracts or task orders), physically completed within the last five (5) years: (1) Project name and location, (2) Cont ractor Role (Indicate if prime, subcontractor or a joint venture, partnerships include % of work performed by prime contractor, sub etc.), (3) Contract Type (fixed price, Cost Plus Fixed Fee, etc.), (4) Brief Project Description. Provide text describin g the Scope of Work, technical approach and complexities, etc., (5) Customer (name or type). Projects should be selected with particular emphasis on those that illustrate proficiency and expertise in investigation and remediation of hazardous waste, site cleanups, OE, etc. The Statement of Qualifications should be in su fficient detail to allow a reasonable evaluation of the firms capabilities and experience on complex projects of the type noted. ORGANIZATIONAL APPROACH: Describe your organizational approach and whether you propose to use your own resources solely, a join t venture, a teaming arrangement or other subcontract arrangement and state your business size. Submit the actual organizational charts for the prime and, if the contract is to be a team arrangement, submit organization charts for each team member. Provi de mailing addresses and office locations of the corporate headquarters, regional offices and other offices for each team member. Personnel on the organizational chart, including support staff and team member/subcontractors, should be identified by name, discipline, task area, and company and office location. The page limit for the SOQ is 30 pages, 12 pt. font (minimum). Any information provided in excess of the 30 page limit will not be considered in the evaluation. For the purposes of this section, a completed project will be defined as cost reimbursable contracts and task orders that are physically complete and for which final payment has been requested or made and fixed price contracts where final payment has been made.
