THE SOURCES SOUGHT NOTICE HAS BEEN RELEASED TODAY AND IS POSTED ON THE PROCUREMENT GATEWAY'S WEBSITE AT: http://progate.daps.dla.mil/home/ DEFENSE SUPPLY CENTER PHILADELPHIA (DSCP) DIRECTORATE OF MEDICAL MATERIEL SOURCES SOUGHT NOTICE for SURGICAL GRADE INSTRUMENTS Amendment 0001 Issue Date: February 18, 2004 Reference the Request for Samples/Literature to locate potential sources for the standardization of surgical grade instruments, within the Department of Defense (DoD) and the Department of Veterans Affairs (VA), to secure the best possible products and prices for their respective hospitals and clinics, worldwide issued February 9, 2004. REVISED – Price Quotations: On page 1, ADD the following sentence to the end of the paragraph: ”The Technical Evaluation Factors listed on Page 5 completes the Technical Evaluation and submitters will not be subject to additional Technical Evaluation Factors during the request for price quotations.” REVISED - Submission Deadline: On Page 1, under Submission Deadline, DELETE “Monday, March 1, 2004 and SUBSTITUTE “Wednesday, March 10, 2004.” REVISED - General Information: A. On Page 2, under General Information, DELETE paragraph 1 in its entirety and SUBSTITUTE the following: “1. Each submitter must provide the identified quantity for all 74-item types referenced. Alternate offers, for all 74-item types referenced, from a different manufacturer are acceptable as long as they are clearly identified as a separate offer. For example, a submitter may submit a proposal containing the identified quantity for all 74-item types from Manufacturer X and in addition submit an alternate proposal containing the identified quantity for all 74-item types from Manufacturer Y. Submitters providing alternate proposals must ensure that the alternate proposal (including products being supplied under the alternate proposal) is clearly identified as the alternate proposal. Submitters providing alternate proposals must submit the identified minimum quantity of the sterilization containers and stringers for each proposal. Submitters are also permitted to commingle manufacturers’ products as shown in the below two examples: Example 1 – Commingled Manufacturers’ Product by Surgical Instrument Groups: A submitter may provide all types and quantities of Scissors from Manufacturer A and provide all types and quantities of Needle Holders from Manufacturer B. Submitters must also submit the identified quantity for the sterilization containers and stringers. Example 2 – Commingled Manufacturers’ Product within a Surgical Instrument Group: A submitter may provide the Curved Metzenbaum Scissors, at least 5”, 2 each from Manufacturer A and provide the Curved Metzenbaum Scissors, at least 8”, 2 each from Manufacturer B. Failure of the submitter to provide all item types in the correct quantities will make the submitter ineligible to receive the BPA.” B. On Page 2, under General Information, ADD the following to the end of paragraph 2: “Alternate proposals will be evaluated independently of each other. For example, a failure under the initial proposal doesn’t mean a failure under the alternate proposal.” C. On Page 2, under General Information, paragraph 3, DELETE “Multiple submitters providing identical manufacturer’s items:” and SUBSTITUTE “Multiple submitters providing identical manufacturer’s items (also applies to alternate proposals): REVISED – Initial Screening of each Submission: A. On Page 3, under Initial Screening of each Submission, under Paragraph 1, DELETE “Monday, March 1, 2004 and SUBSTITUTE “Wednesday, March 10, 2004.” B. On Page 3, under Initial Screening of each Submission, under Paragraph 3, DELETE the first sentence and SUBSTITUTE “All instrument samples have the required dull, unpolished finish (also known as satin finish).” REVISED – Surgical Instrument Groups – Product Specific Information: Reference Page 18, Abdominal Self-Retaining (BALFOUR) Retractors. DELETE paragraph B.1 and B.2 and SUBSTITUTE: “B. Type and Quantity 1. Self-Retaining Retractor - Pediatric set, 1 set ea. 2. Self-Retaining Retractor - Regular set, 1 set ea.”
