Description
The contractor is to provide contract summer camp staff to operate the program with six (6) counselors, one (1) program coordinator, and one (1) Aquatics staff for a full-day school-age program. The program coordinator for the summer camp program for school-age will arrive no later than 17 May 04 and counselors will arrive no later than 07 Jun 04. All contracted summer camp staff will depart no earlier than 21 Aug 04. The actual summer camp program begins on 14 Jun 04 and ends on 21 Aug 04. The second part of this contract is to provide contract child development staff to operate with six (6) counselors and one (1) program supervisor. The contracted child development program staff will be (as needed) in the following time periods starting 10 May 04-3 Sep 04 (17 weeks). Staff will work in the school-age program and child development program 40 hours per week with a minimum of two (2) consecutive days off per week. Programs planned shall be appropriate for children ages 6-12 years of age enrolled in the summer camp and 3-5 years of age, activities held indoors and outdoors, and conducted using the youth facilities, school-age towers, and child development facilities on base. The proposal should include developmental appropriated activities for youth ages six (6) to 12 years of age in school-age care and children ages 6 weeks to 6 years of age in the child development program: All contractor staff must meet standard youth activity guidelines in reference to freedom of communicable disease, drug abuse, and child abuse. (1) The contractor must comply with requirements for a review of personnel and security records to include a FBI fingerprint check and State Criminal History Repository (SCHR) checks or residences listed on employment and security application. At a minimum SCHR check, communication in writing will be made with each state, providing full identifying information concerning each applicant and requesting confirmation that the individual has not been convicted in that state of a sex crime, an offense involving a child victim, substance abuse felony, or a violent crime. Contractor employees will submit a completed Standard Form 85P, ?Questionnaire for Public Trust Positions?, which will be provided to the Contractor by the NAFI. The reason for request will be identified as OTHER and annotated as CHILD CARE. The FD Form 258, ?FBI Applicant Fingerprint Card? and the completed Standard Form 85P shall be forwarded for processing to: Defense Investigative Service Personnel Investigative Center Attn: Air Force Liaison 881 Elkridge Landing Road Linthecum MD 21090 (2) If an SCHR check is not returned by the state(s) within 60 days, a final determination may be made based on the FBI fingerprint check and available background information. While background checks are processing, staff may be provisionally hired as long as they are within the line-of-sight of an individual with a satisfactorily completed background check. Contract employees have the right to obtain a copy of the background check report and to challenge the accuracy and completeness of the information contained in the report. Contractor employees who have previously received a background check must provide proof of the check or obtain a new one. (3) The contractor staff has had a physical examination within the last 3 years and has the required immunizations for the area where they will be assigned. Personnel assigned to YAB SAP will be screened for medical/emotional or special needs related conditions that may require special medical support or modified housing. d. All counselors shall be trained to administer basic first aid and cardiopulmonary resuscitation (CPR) and be certified as qualified by a competent authority such as Red Cross. Counselors must be trained in Red Cross Basic Life guarding and/or emergency Water Safety. This training will be provided to the counselors by the Contractor, at no cost to the YAB SAP, prior to the counselors working in the field.
