- This is a synopsis for commercial items prepared in accordance with the format under Simplified Acquisition Procedures in accordance with FAR Part 13.5, as supplemented with additional information included in this notice. The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-27. This acquisition is 100% small business set-aside and the North American Industry Classification System (NAICS) code is 561599 with a small business size standard of $6.0 million. It is the intent of National Library of Medicine (NLM) to contract with a person trained in hand book and paper conservation techniques, to work on-site, at the Library, for a minimum of 25 and a maximum of 125 hours per month, to do repairs to paper and bindings in the rare book collection, construction of phase boxes, portfolios, and other housings, preparation of materials and record-keeping for off-site conservation and microfilming, and design and construction of supports for exhibit of materials. Demonstrated training and experience in hand conservation treatments and evidence of ability to work independently and to make appropriate decisions concerning treatment of early books, manuscripts, and graphic works on paper are required. The purchase order term will be April 1, 2004 through March 31, 2005 with options for two (2) successive one-year periods. All work shall be performed in accordance with accepted principles of rare book conservation and in adherence to American Institute for Conservation (AIC) "Code of Ethics and Guidelines for Practice" (available at http://palimpsest.stanford.edu/aic/pubs/ethics.html). The FAR provisions and clauses sited herein are incorporated by reference into this solicitation. Offerors are advised to propose in accordance with the provision of FAR 52.212-1, Instructions to Offerors-Commercial Items. Offerors are advised to provide with their offer a completed copy of the following provisions: FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (http://www.arnet.gov/far/). The following FAR clauses apply to this acquisition: FAR 52.212-4, Contract Terms and Conditions-Commercial Items; FAR 52-212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. Request for Quotations (RFQ) Number NLM 04-080-UHP will be available on or about March 16, 2004. Request for RFQ-NLM-04-080-UHP may be obtained by writing to the National Library of Medicine, Office of Acquisitions Management, 8600 Rockville Pike, Building 38A, Room B1N20, Bethesda, Maryland 20894; Attention: Uyen H.
