Description
This is an Amendment, which will be identified as number three (3) for Solicitation FA4613-04-Q-0008. Please note the following changes: 1. The Investigative Design Proposal Response Date has changed from 12 March 2004 to 19 March 2004. 2. PHASE III: INSTALLATION shall now include the following changes, which directly affects the Statement of Work: The sheetrock ceiling joints are identified as an asbestos containing material (ACM) reference Attachment 3. The Contractor must provide personnel (trained to work around asbestos/lead based paint materials) to cut the necessary penetration holes in the ceiling to accommodate the designed new ductwork, in a manner whereby friable asbestos is not released in the process. Contractor personnel will avoid disturbing the sheetrock material during the process of system installation, to prevent exposure to asbestos. Lead based paint has also been detected in the building and it will be incumbent upon the Contractor to abate any lead based paint that poses a contamination threat (Attachment 3) to personnel installing the dust collection system equipment. The Contractor shall read Attachment 3 and notify 90 CONS if it is suspected that other installation processes are anticipated to disturb either lead based paint or asbestos. The Contractor shall develop an asbestos/lead based paint abatement/worker safety plan and submit it for base approval, as a part of the Phase II submittal. 3. Please note the following CLIN 0004 has been added to this RFQ.
