- Executive Order 12564 establishes standards for a Drug-Free Federal Workplace and requires agencies to have Employee Assistance Programs providing high-level direction, education, counseling, rehabilitation, referral and coordination services with community resources. The United States Holocaust Memorial Museum (USHMM) requires the services of an Employee Assistance Program provider for a base year plus 4 one-year option periods. The Contractor shall provide employees and family members a broad range of counseling services for any personal or work-related conflict or concern. The range of concerns to be covered includes any emotional, behavioral, mental or personal concern, especially those related to alcohol or drug abuse, which may adversely impact on the employee?s job performance, attendance, or conduct. This would include service to a total of approximately 375 employees of the U.S. Holocaust Memorial Museum in the Washington, D.C. metropolitan area, including a facility site in Linthicum, Maryland, and four regional offices located in New York, New York; Los Angeles, California; Boca Raton, Florida, and Highland Park, Illinois. This solicitation is for commercial services in accordance with FAR Part 12. This procurement shall be 100% small business set-aside. The NAICS Code for this procurement is 624192, Other Individual and Family Services. In order to be considered for an award, offerors must be registered in the Central Contractor Registration (CCR); (FAR 4.1102). To register your company on the CCR System, go to http://www.ccr.gov. It is anticipated that this requirement will be awarded as a fixed price contract. Award will be made to the responsive, responsible offeror after evaluation of technical criteria and price. It is estimated that evaluation and award will be made on or about June 1, 2004 The delivery of proposals will be by 12:00 p.m. (EST), April 5, 2004, to Division of Procurement, Attn.: P. Smith, U.S. Holocaust Memorial Museum, 100 Raoul Wallenberg Place, SW Washington, DC 20024-2150.
