Description

This is not a solicitation announcement. This is a SOURCES SOUGHT synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classifications (hubzone, 8(a), small, small disadvantaged, veteran owned, service disabled veteran owned, woman owned, or large business relative to 2002 NAICS Code 423450 (size standard 100 employees)(replaced 1997 NAICS Code 421450). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions - see submittal requirements below. After review of the responses to this sources sought synopsis, the Government intends to publish a solicitation online in http://www.fedbizopps.gov/ and in http://www.bos.oamm.va.gov/. Responses to this sources sought synopsis are not adequate response to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. The Department of Veterans Affairs, Network Business Center, is seeking sources for a potential consolidated contract to provide all labor, materials, equipment, and transportation necessary to supply Custom Sterile Surgical Packs for fifteen (15) various Government Healthcare Facilities located within VA VISN 16, VISN 18, VISN 19, VISN 20, VISN 21, VISN 22 and DoD Facilities within the same geographical boundaries. There shall be the possibility of adding and/or deleting facilities within these VA geographical parameters at later dates throughout the duration of the contract. The intended contract will be for a base year with the Governments right to extend the contract in one-year increments for four option years. Response to this notice should include company name, address, telephone and facsimile numbers, point of contact, email address, size of business pursuant to North American Industrial Classification System (NAICS) and must respond to the following questions: (1) Is your business large or small? (2) If small, does your firm qualify as a small-emerging business or a small disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act. (4) Is your firm a certified Hubzone firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service Disabled Veteran Owned or Veteran Owned business? This is not a request for proposal and in no way obligates the Government to award any contract. Responses to this notice shall be mailed, emailed or faxed to the attention of Larry R.
