Description

Contractor shall provide all necessary labor, supervision, equipment, and materials to furnish, deliver and install concrete grave liners at National Memorial Cemetery of Arizona, 23029 N. Cave Creek Road, Phoenix, AZ 85024-7500. Contract covers base period (date of contract award) through September 30, 2004, with up to four 1-year options to extend the term of the contract through September 30, 2008. This acquisition is a total (100%) set-aside for small business concerns. North American Industrial Classification System (NAICS) Code 327390 is applicable. Request for a hard copy of the solicitation must be in writing to Ms. Michelle Cruz, Purchasing Agent, and accompanied by a check or money order in the amount of Twenty-Five Dollars ($25.00) (non-refundable), payable to ?U.S. Department of Veterans Affairs.? Please reference Solicitation Number IFB 786-187-04 on remittances. No telephone requests will be accepted. Hard copy solicitations will be issued via the ordinary mails on or about May 14, 2004. The solicitation may also be obtained FREE OF CHARGE, from VA?s Business Opportunities System via the Internet at the following URL address on or about May 14, 2004: http://www.bos.oamm.va.gov/solicitation?number=786-187-04. Numbered note 1 applies. Bidders are responsible for all amendments.
