- Description
- This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-04-T-0029 is issued as a request for quotation (RFQ). The following items are specially designed microcatheters, microguidewires and embolic materials essential in embolization/angioplasty procedures. CLIN 0001, QTY: 10 EA: Agility (R) Hydrophilic Steerable Guidewire, Agility 16 Standard 175cm, Cordis P/N 614541; CLIN 0002, QTY 10 EA: Agility (R) Hydrophilic Steerable Guidewire, Agility 10 Standard 195cm, Cordis P/N 614178; CLIN 0003, QTY 10 EA: Prowler (R) Small Profile Infusion Microcatheters, Prowler 14, Cordis P/N 606151; CLIN 0004, QTY 10 EA: Prowler (R) Small Profile Infusion Microcatheters, Prowler Plus, Cordis P/N 6062510FX; CLIN 0005, QTY 1 EA: Trufill (R) NBCA Liquid Embolic System 211, Trufill Liquid Embolic System, Cordis P/N 631400. The solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-22. Commercial Items (JAN 2004), the following factors shall be used to evaluate offers: 1) Technical Capability, 2) Past Performance, and 3) Price. Past performance and technical capability have equal importance to cost or price. Prior to Contract Award, the Government reserves the right to review the past performance of the contractor solely for the purpose of ascertaining its qualifications to perform the requirements of the contract.
