Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this Request for Quotation is W91YU0-04-T-0013. This solicitation is issued on an unrestricted basis under NAICS number 339112. DESCRIPTION: Thirty (30) eac h Medfusion 3500 Syringe pumps or equivalent, to include: A/C power supply, rotating pole clamp, operations manual and a one (1) year parts and labor manufacturer’s limited warranty. Delivery of all thirty (30) pumps is required no later than three we eks from receipt of order. Minimum salient characteristics as follows: User-Defined Delivery Libraries; Volume Over Time Delivery Mode; Rapid Alarm System (less than 5 minutes at 1.0ml/hr with 60cc syringe); flow rate in range of 0.01 ml/hr to 1130 ml/hr ; internal battery capacity about 10 hours at 5.0 ml/hr with 60 ml syringe; able to work in MRI rooms with static magnetic fields up to 150 gauss; programmable upper/lower limits for dose, rate, bolus and loading dose. Offers responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Officer-Commercial Items. As stated in this clause, the Govern ment intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror’s initial proposal should contain the best terms from a price and technical standpoint. In accordance with FAR 52.212-3, evaluation factor s are: technical capability, price and past performance. However, the Government reserves the right to conduct discussion if later determined by the Contracting Officer to be necessary. Offerors shall include a completed copy of provision FAR 52.212-3, “Offeror Representations and Certifications Commercial Items”. An authorized representative of the offeror must sign these representations and certifications; the offeror shall also provide its Commercial and Government Entity (CAGE) code, Contr actor Establishment Code (DUNS number), and Tax Identification number (as required). The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, “Contract Terms and Conditions Commercial Items”. FAR Cla use 52.212-5, “Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items” (clauses 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-33) and DFAR Clause 252.212-7001 “ Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items” (clauses 52.225-7001 and 52.232-7003) apply to this acquisition.
