Description

Change to Stock # This announcement constitutes a Request for Quotation (RFQ). No formal, written solicitation will be issued. The United States Senate Office of the Sergeant at Arms (SAA) has a requirement for Tyvek Suits Stock #: DuPont - TY 122S WH, in assorted sizes 2X, 3X, 4X, and 5X. The most used sizes are 3x, 4x, and 5x. The SAA intends to award a firm fixed price indefinite delivery indefinite quantity (IDIQ) type contract, to the responsible Offeror with acceptable past performance, whose quotation, conforming to the solicitation as provided herein provides best value to the SAA. The period of performance will be from the date of award through September 30, 2005 for the base period, with one (1) twelve month option period (October 1, 2005 through September 30, 2006). The SAA will issue delivery orders under the contract which shall state the actual quantities per size. All deliveries shall be FOB Destination to the United States Senate, Washington DC 20510 and made within five (5) business days of the issuance of a delivery order. Award is anticipated within thirty (30) days of receipt of quotations. Quotations are due to the SAA POC no later than May 25, 2004 at 12:00 (Noon) EST and must be submitted electronically via e-mail to: acquisitions@saa.senate.gov. The subject line of the e-mail message shall be Tyvek Suits 2004-R-046. The Quotation must contain: 1) Company Name, Address, Point of Contact, Telephone Number, E-mail address, DUNS Number, and Tax ID Number; 2) Pricing - unit prices (including single item prices and quantity discounts). Estimated maximum quantities are 13,000 Tyvek suits for the base period and 13,000 Tyvek suits for the option period; and 3) Three past performance references for contracts/purchase orders of similar products within the last two (2) years. Past performance references must provide: a) Name of the Customer/Company; b) Name and phone number of the primary contact with Customer/Company; and c) contract/purchase order value. Access by the SAA to information in any files attached to the quotation is the responsibility of the submitting party. The SAA is not responsible for any failure to access information. The Senate or the SAA will not be responsible for any costs for the preparation of responses to this announcement. All information is included in this announcement.
