Description

The VA VISN 22 Network Business Center intends to negotiate a commercial (FAR Part 13) sole source contract with Advanced Medical Optic?s Inc, 1700 E ST Andrew PL, Santa Ana, CA 92799 under the authority of FAR 6.302-1. Only one responsible source and no other supplies or services will satisfy agency requirements. The Contractor agrees to install for the VA Loma Linda Healthcare System the complete AMO Compact System the system includes the following items: 1) Remote qty 1; 2) Footpedal qty 1; 3) I/V Pole qty 1; 4) Cart qty 1; 5) Bipolar Cord, Pencil Connector qty 2; 6) Bipolar Forcep, Curved Iris qty 2; 7) Solo 1/A Set; Straight Curved, 45 Deg qty 2; 8) Electric Oscillating Vitrectomy Cutter qty 1; 9) Limbal Infusion Needle qty 1; 10) Coaxial Irrigation Sleeve qty 1; 11) Sterilization Tray 1 ? medium qty 2; 12) Sterilization Tray ? Medium for EOVOC qty 1; 13) Mayo Tray qty 1; 14) Tip Wrench 14 qty 1; 15) Bio-Manual Hand Pieces with accessories Interested persons may identify their interest and capability to respond to the requirement by submitting their request in writing with complete name, address, telephone and fax numbers, E-mail address and point of contact.
