Description

This is Nonprofit Small Business acquisition in accordance with the Public Health services Act Public Law 102-321 as amended Part E Title, Section 561-565. The Center for Mental Health (CHMS) is soliciting proposals to provide training and technical assistance for the Statewide Family Networks Program, the Statewide Consumer Network Program for the Government Performance and Results Act (GPRA). In addition, data collection, data processing, analysis and final GPRA data report to the Center for Mental Health Services for both Network Programs. The North American Industry Classification for System (NAICS) Code for this acquisition is 541820. The estimated issuance date of the RFP 280-04-0085 is June 8, 2004 with proposals due approximately July 8, 2004. The solicitation may be obtained in either of two ways; by mail, or electronically, by downloading a file from the World Wide Web at http://www.samhsa.gov/contracts/contracts.html Files will not be made available on the World Wide Web until on or after the estimated issuance date. Once downloaded, you should determine if you have a continuing interest in the RFP. If so, you must either write the individual identified above that you wish copies of amendments to the RFP (if any) sent to you by mail or check the Web regularly for amendments, which will also be posted there and can be downloaded electronically. You may choose any of the two methods for obtaining amendments. Note, however, that if you rely on the Web for obtaining amendments, at this time we have no way to assure, and will therefore assume no responsibility for assuring, you have downloaded these amendments. All RFP requests must cite the RFP Number. Requests for copies of the RFP to be mailed must be made in writing. Fax requests will be accepted at (301) 443-0019 but please confirm by phone that your fax request has been received. Requests will be filled on a first-come, first-served basis until the supply is exhausted. Telephone requests will not be honored. This is Nonprofit Small Business acquisition in accordance with the Public Health services Act Public Law 102-321 as amended Part E Title, Section 561-565, all responsible sources may submit a proposal, shall be considered by SAMHSA/CMHS.
