Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) Sub part 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested THERE WILL BE NO WRITTEN SOLICITATION ISSUED. The solicitation number for this Request for Proposal is RFP 648-65-04. This solicitation is 100% set aside for Small Business. This solicitation is issued on a restricted basis as a total small business set aside under NAICS Number 453998. DESCRIPTION: Grasspave" OR "Marker Grid" (Brand name or equivalent) is a plastic product in the form of an expanded diamond shaped grid that is used in landscaping applications where high traffic or other considerations require a non visible means to support turf. In this application, the product is installed under a row of grave markers, eliminating the need to re-position the grave markers as they settle. The product must be fabricated in a manner to allow sections of the roll to be unfastened then reattached to keep the integrity and strength of the product. This will allow future (second/occupied) burials to be completed without a break in the line to avoid future settling. The product should be manufactured with ridges on two sides to hold grave markers in alignment. Estimate Quantity is 124,223 per SF Unit of Grass Pave Brand name or equal. 1. Evaluation Factor 50% Contractor?s demonstrated ability to meet the desired characteristics of the product in order to achieve maximum results of renovations and burials. a. The product can be unfastened then reattached during burials and renovations, keeping the integrity of the initial placement intact. b. The product has a ridge for the top and bottom of the grave marker to assure alignment on slopes and prevent slippage due to soil conditions or equipment wear. c. The product is available in rolls in varying lengths from 20 feet to 160 ft. which will allow a variety of installation options. d. The product width closely measures the width dimensions of a typical grave marker of 12 inches, which allows efficient placement, and aligning of the product. 2. 30% Price 3. 10% Past performance: Identify all Federal, State, and local government contracts and private contract of similar scope, size, and complexity that are ongoing, or have been completed within the past three years. Provide a list of references pertaining to those contracts. Information to be provided must include contract names and phone numbers. In addition, information must be provided that evidences customer satisfaction with an offeror?s compliance with and fulfillment of the requirements of previous contracts. 4. 10% Management Capabilities and Approach: Describe management methods to assure the provision of prompt and quality service including methods for corrections of errors or procedures required by the Department of Veterans Affairs. Clause 52.212-1, Instructions to Offeror-Commercial Items. As stated in this clause, the government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the Offeror/s initial offer should contain the offeror/s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussion if later determined by the Contracting Officer to be necessary. Offerors shall include a completed copy of provision 52.212-3. Offeror Representations and Certification?Commercial Items. An authorized representative of the offeror must sign these representations and certifications; (4) Contractor Establishment Code (DUNS number) and Tax Identification number. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract terms and Conditions?Commercial Items.
