Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with the commercial item procedures in FAR Part 12. The solicitation is being issued as a Request for Proposal (RFP). The solicitation number is 246-04-01488. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-23. The NAICS is 423450 and the small business size is 100 employees. Items will be purchased and delivery will be made to the Department of Veterans Affairs Medical Center, 508 Fulton St, Durham, NC 27705 and the Department of Veterans Affairs Medical Center 1201 Broad Rock Blvd, Richmond, VA 23249. Offeror's proposals are hereby being requested in response to the requirements under a firm-fixed price contract and shall be submitted on offeror's letterhead or bid form and shall contain the name, title, and signature of the person authorized to submit proposals on behalf of the contractor. Proposals are to be provided for the following: two diagnostic and therapeutic catheter systems to be used exclusively in electrophysiology. Pricing shall be provided separately for each medical center. Line item 1 shall be Durham Medical Center's system and line item 2 shall be Richmond Medical Center's system. The systems must utilize navigational mapping through real time data processed to be viewed as three dimensional, color coded cardiac maps and precise real time tracking of catheter location and diagnosis. Systems' accuracy is to be within 1mm. The catheters used with the systems shall map and ablate using the same catheter with temperature ablation. The systems shall utilize a 8mm catheter to allow for longer lesions.The systems should also utilize a 4 and 8mm catheter to allow for better tissue contact and better maneuverability throughout the heart.The systems shall have multi-electrode mapping capabilities. The systems must have the ability to integrate CT scans and MRIs for more accurate mapping.The systems must have deflectable tips.The systems must have the ability to fully integrate with the Steriotaxis system. The systems provided must include all components necessary to make the systems fully functional including but not limited to cables, monitors, printers, software, etc. Contractor must also provide staff training at the Medical Centers. The Richmond Medical Center has an existing system that is to be traded-in. Trade-in value and removal of the system is to be considered when preparing proposals. All responses must include the following: (1) descriptive literature and product number for the system offered proving the system offered meets the above requirements (2) delivery time after receipt of order (3) cost, cost is to be FOB destination. The following provisions and clauses apply to this solicitation: 52.212-1, Instructions to Offeror-Commercial Items, 52.212-4, Contract Terms and Conditions-Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, 52.233-2 Service of Protest, 852.233-70 Protest Content, 852-233-71 Alternative Protest Procedure. Award will be made to the offeror whose product meets the requirements listed above and is most advantageous to the Government. The Government intends to evaluate the information received and award without discussions. Offers including the pricing information are to be submitted on company letterhead and include the information required in provisions 52.212-1 and a completed copy of the provisions 52.212-3, Offeror Representations and Certifications-Commercial Items. In accordance with FAR 52.204-7 award will be made only to a contractor/company that is registered in the Central Contractor Registration (CCR). Electronic copies of the referenced provisions and clauses are available on the internet at www.arnet.gov/far and www.va.gov/oa&mm/vaar/ or fax a request to 757-728-3132. Quotations and all required information must be received by 4:00pm local time June 25, 2004. Any questions regarding this solicitation must be in writing and received in this office no later then June 17, 2004. Quotations shall be submitted to Barbara Simmons, Contracting Officer (90C), VA Medical Center, Hampton, Va. 23667. No faxed offers will be accepted Solicitation number 246-04-01488 shall be listed on the documents submitted for identification purposes. Only offers prepared in accordance with this solicitation shall be considered.
