Description

This is a commercial item solicitation using FAR Part 12 and 15 for providing Community Based Primary medical care services to eligible veterans beneficiaries for the VA Central Iowa Heahtlh Care System in the Fort Dodge, Iowa area and surrounding counties. The required services shall be all inclusive; i.e., health care provider, supples, etc. as this is a capitated contract for Primary Care and fee for service for Mental Health. Estimated average annual primary care enrolled is 4,000 with mental health around 3,000. Contractor shall (1) meet all applicable State and Federal requirements or standards; (2) meet standards set forth in the JCAHO Accreditation Manual for Hospitals or have current Rural Health Designation. Availability of ancillary support services located with the general vicinity is desirable. Base year from October 1, 2004 through September 30, 2005 with four (4) one year renewal options. Issue date o/a July 1, 2004. Closing date o/a August 1, 2004. This acquisition is subject to the availability of funds.
