Description

Provide primary care and mental health outpatient care services in the Wisconsin Rapids, Wisconsin area for the Department of Veterans Affairs Medical Center, Tomah, Wisconsin. The period is October 1, 2004 through September 30, 2005 in accordance with all specifications, work statement, terms and conditions contained therein. Electronic version and hard copy of solicitation should be available on or about May 28, 2004. Electronic solicitation may be obtained FREE at the following URL: http://www.bos.oamm.va.gov/. Requests for hard copy solicitation must be in writing to the Contracting Officer at the GLAC and accompanied by a check for $10.00 made out to the Department of Veterans Affairs. No telephone requests will be accepted. Any amendments issued to this solicitation will also be available at the URL address. Offerors are that they are responsible for obtaining amendments. Offerors should contact michelle.ranes@med.va.gov or VA Central Office 866-594-0508 regarding any problems downloading the solicitation from the website.
