Description
THE PURPOSE OF THIS AMENDMENT, THE SECOND AMENDMENT, TO W914NS-04-Q-0173 IS TO CLARIFY TO ASPECTS OF THE SPECIFICATION AND TO PROVIDE AN UP TO DATE PAST PERFORMANCE QUESTIONNAIRE. 1. The solicitation document and incorporated provision and clauses are those in effect through Federal Acquisition Circular 19, effective 06/8/2004 and DFARS Change Notice 20040608. 2. This is a solicitation for a NEW boat. Offerors for reconditioned or used vessels will be rejected as technically unacceptable. Catamaran designs will be rejected as technically unacceptable. A water jet propulsion system in lieu of a propeller propulsion system may be offered as part of a technical solution. Offerors may offer only one proposal and must elect which type of propulsion system they wish to offer. 3. Disregard the Past Performance questionnaire that was attached to the original solicitation. The Past Performance questionnaire that should be used is as follows: PAST PERFORMANCE QUESTIONNAIRE INSTRUCTIONS SOLICITATION NUMBER W914NS-04-Q-0173 1. Please complete this questionnaire. Handwritten responses are sufficient. Explanation of codes: CODE PERFORMANCE LEVEL Blue (E) EXCEPTIONAL - The contractor?s performance meets contractual requirements and exceeds many requirements. Green (VG) VERY GOOD ? The contractor?s performance meets contractual requirements and exceeds some requirements. Yellow (S) SATISFACTORY - No problems exist or only minor problems for which solutions are in hand. Black (M) MARGINAL - Problems exist for which there is doubt whether the identified solution is adequate but the problem appears to be within the contractor?s ability to solve. Red (U) UNSATISFACTORY - Serious problems exist which may be outside the contractor?s ability to solve. The contractor is in danger of not being able to satisfy contractual requirements and timely recovery is not likely. Grey (N) NOT APPLICABLE - Unable to provide a score. 2.
