- Description
- The Federal Bureau of Prisons, Field Acquisition Office, Grand Prairie, Texas, intends to issue solicitation RFQ 30703-032-4 for the provision of Eye Care Specialist Services to be performed by a licensed Optometrist or Ophthalmologist for inmates held in the custody of the Department of Justice, Federal Bureau Prisons, Federal Correctional Institution, Jesup, GA. The Government anticipates awarding an indefinite-delivery, requirements-type contract with firm-fixed unit prices. The requirement for the service will not exceed 30 hours per week. A session will be defined as one-hour. It is estimated the contract will include two - seven hour sessions per month and 168 sessions per contract year. The contract period shall consist of one 12-month base year and four 12-month option periods for renewal. The estimated quantities for the base year and four option years are as follows; Base Year - 168-one hour sessions, Option Year 1 - 168 one hour sessions, Option Year 2 - 168 one hour sessions, Option Year 3 - 168 one hour sessions, Option Year 4 - 168 one hour sessions. The contractor will normally see five patients per one hour session. The contractor will be responsible for performing eye examinations and refractions for inmates at FCI Jesup, as well as other services as follows; the basic exam will include refraction, visual exam of the eyes, including funduscopy and tonometry. Other duties include verification of eyeglasses upon receipt, ordering eyeglasses based on the findings of the examination, fitting of eyeglasses, checking for verification of accuracy of prescriptions prior to dispensing to the patient, making adjustments prior to dispensing to the patients, evaluating ocular disease states, providing treatment and making recommendations for further evaluations or tests, including but not limited to visual field exams, fundus photography and reports, conjunctival and corneal foreign body removal, informing the appropriate staff of any discrepancies or equipment needs. The contractor will also be responsible for furnishing some equipment and supplies, as specified by the Government. The solicitation will be available on or about July 26, 2004, and will be distributed solely through the website at www.fedbizopps.gov. All future information concerning the acquisition, including solicitation amendments will be distributed through the website. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The anticipated date for receipt of quotes is August 26, 2004. All responsible sources may submit a quote which will be considered. Faith-Based and Community-Based Organizations have the right to submit quotes equally with other organizations for contracts for which they are eligible.
