- Correction: This action originally publicized in the FedBizOps dated 05/24/04 is change in procurement method and response date as follows: ???This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued.??? Solicitation No. RMN00040054 herein is cited as a Request for Proposal (RFP). The commercial items policies of FAR Part 12 will be used in conjunction with the simplified acquisition procedures of FAR Part 13. Solicitation document and incorporated provision and clauses are those in effect through Federal Acquisition Circular 97-20. This acquisition is a small business set-aside. NACIA Code: 237110, Small Business Size Standard: 500 employees . (Description): Item Description Quantity Bid Amount 1. Structural Concrete AASHTO M43, No. 57 1 Lot $_________ Item Description: The total bid schedule concrete quality of 725 Cubic Yards includes approximately 200 cubic yards for concrete headwalls and blankets; approximately 250 cubic yards for a 3 barrel ??? 8 ft. X 8ft. X 113 ft. concrete box culvert and a 6 barrel ??? 10ft. X 10ft. X 93ft. concrete box culvert. The concrete shall be Class A (AE) with a minimum compressive strength of 3,000 psi (20.7 MPa), the listed materials shall be certificate of compliance in accordance with AASHTO M43, No. 57 as specified under FP-96, Section 552 ??? Structural Concrete, Standard Specifications (FP-96) for Construction of Roads and Bridges on Federal Highway. Contractor shall submit a concrete mix design on FHWA Form 1608 or similar to the Navajo Region, Division of Transportation (NRDOT), P.O. Box 1060, Gallup, NM 87301, Attention: Mr. Alfred Myron, Materials Engineer, Telephone No. (505)863-8447, Fax No. (505) 863-8335. The concrete design shall include Material Certification for all aggregates, Portland cement, fly ash (if needed), and admixtures. The concrete mix design shall be reviewed and approved by the NRDOT, Division Manager before any deliveries or orders are made. If the concrete cannot be delivered within the time limits provided in Table 552-2, the Contractor shall deliver by the dry batched method or utilize a portable batch plant on site; unless the Contractor submits a written request to the Contracting Officer (for review and approval) a different method which shall meet all the contract and specification requirements. The Bureau of Indian Affairs field personnel will order (by telephone) the quantity of concrete and ate of delivery to the project site on an ???As Needed Basis???. Waste concrete shall be disposed off by the Contractor in accordance with the Clean Water Act requirements. Absolutely no concrete shall be wasted on the project site. Deliver FOB destination to Project N5000 Construction Yard located approximately 27 miles south of Shiprock, New Mexico on Hwy 491 (formally known as US Route 666) and 4 miles west on Hwy No. 34, in the vicinity of Tocito, New Mexico, on the Navajo Nation. Award will be made at a fair and reasonable price to the responsive and responsible bidder whose bid conforming to the requirements herein will be the most advantageous to the Government, price and other factors considered.
