- Description
- The United States Government represented by the U.S. Agency for International Development intends to procure medical equipment described in: http://www.usaid.gov/sv/procurem/medequp.htm This is a combined synopsis solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitations will not be issued. Solicitation No. 519-04-Q-005. This solicitation document incorporated provisions and clauses in effect through Federal Acquisition Circular 01-20(02-23-2004). The following FAR clauses apply to this solicitation and are incorporated by reference: FAR 52.212-1(Jan 2004); FAR 52.212-2(Jan 1999); FAR 52.212-3(June 2003); FAR 52.212-4 (Oct 2003);52.212-5(Oct 2003). Offerors must execute and submit the requirements at 52.212-3, Offeror representations and Certifications-Commercial Items with the offer. PURPOSE: To procure medical equipment for the Ministry of Health (MOH), in El Salvador, Central America for a program on Child Survival and Reproductive Health Care Services which is supported by USAID/El Salvador under the Healthy Salvadoran Activity No. 519-0430. The list of the medical equipment, specifications and requirements can be found at : http://www.usaid.gov/sv/procurem/medequp.htm The authorized Geographic Code for this procurement is 000 (United States). Awards will be placed on or before August 30, 2004. The government intends to evaluate offers and award a firm fixed price contract to the lowest, responsive, responsible offeror. Therefore the initial offer must contain and meet all the Government specifications and requirements. The government reserves the right to conduct discussions if the Contracting Officer determines that discussions are necessary. The Government may reject any or all offers that waive informalities and minor irregularities in offers received. The award decision will be based upon, in descending order of importance, price, technical compliance and quality, warranty terms and past performance. The government's preference is to award one contract for all items. However the government may determine it is more advantageous to place separate orders for line items where there is a significant cost savings available on one or more items. Format: Please use the format at http://www.usaid.gov/sv/procurem/medequp.htm and maintain the sequential order for all items and propose unit and total price for each item. Offers are due at 4:00 p.m. El Salvador time on July 23, 2004. They must be submitted with the name and address of the Offeror and Requests for Quotation (RFQ) No.:519-04-Q-005 to: Mailing address: USAID/El Salvador, Regional Contracting Officer, Urbanizaci?n y Blvd. Santa Elena Sur Antiguo Cuscatlan, La Libertad, El Salvador.
