- This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. THIS REQUIREMENT IS RESTRICTED TO MAINLAND PORTUGAL AND ITS TERRITORIES. The solicitation number for this procurement is FA4486-04-T-0022 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2001-21, Defense Acquisition Circular (DAC) 91-13.Air Force Acquisition Circular (AFAC) 2004-0302. For informational purposes The Standard Industrial Classification Code is 5045. The North American Industry Classification System is 441229. Provision at FAR 52.212-1, Instruction to Offerors Commercial Items applies to this acquisition. Request for Quotation (RFQ) shall be submitted and shall contain the following information: RFQ NUMBER; TIME SPECIFIED FOR RECEIPT OF OFFERS; NAME, ADDRESS; TELEPHONE NUMBER OF OFFEROR; TERMS OF THE EXPRESSED WARRANTY; PRICE, ANY DISCOUNT TERMS AND ACKNOWLEDGEMENT OF ALL SOLICITATION AMENDMENTS (if applicable). Quote shall also contain all other documentation specified herein. Offerors shall include a statement specifying the extent of agreement with all terms, conditions and provisions included in the solicitation. PERIOD OF ACCEPTANCE OF OFFERORS: The offeror agrees to hold the price in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation. EVALUATION/AWARD: IAW FAR 52.212.2, Evaluation-Commercial Items, the Government will award a purchase order resulting from this solicitation to the responsive responsible low offeror or the offer representing the best value when compared to any lower price quotation or offer. All offerors should be CENTRAL CONTRACTOR REGISTERED (CCR). Information concerning CCR requirements may be viewed via the Internet at http://www.ccr.gov or by calling the CCR Registration Centers at 1-888-227-2423. OFFERORS SHALL INCLUDE A COMPLETE COPY OF THE PROVISION AT FAR 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS-COMMERCIAL ITEMS, WITH THEIR OFFER, which can be downloaded from the internet (http://www.farsite.hill.af.mil) Offerors that fail to furnish the required representation information or reject the terms and conditions of the solicitation may be excluded from consideration. Pursuant to FAR Clause 52.212-5 the following clauses are hereby incorporated by reference: FAR 52.222-3 Convict Labor (E.O. 11755), FAR 52.222-26 Equal Opportunity (E.O. 11246), FAR 52.225-1, Buy American Act-Balance of Payments Program Supplies, FAR 52.247-64, Preference for Privately Owned U.S. Flag Commercial Vessels (46 U.S.C. 1241), FAR 52.232-33, Mandatory Information for Electronic Funds Transfer Payment. RESPONSE TIME: REQUEST FOR QUOTATION WILL BE ACCEPTED AT THE 65th CONTRACTING SQUADRON LOCATED AT LAJES FIELD, AZORES, PORTUGAL NOT LATER THAN 2:00 P.M. LOCAL TIME ON 9 July, 2004. FACSIMILE OR EMAIL OFFERS WILL BE ACCEPTED. It is the offeror?s responsibility to confirm receipt of quotations. The point of contact is SSgt Holly Pearlman holly.pearlman@lajes.af.mil 011-351-295-57-1486 The government?s intention is to purchase a currently leased vehicle. The Contractor shall provide all personnel, equipment, materials, and vehicles necessary to provide a refuse collection vehicle. The vehicle shall be delivered to the 65th Logistics Readiness Squadron, T-216, Lajes Field, Azores, Portugal in a safe and operable condition. The Contractor shall provide a contract manager who shall be responsible for the completion of this contract. The name of this person shall be designated in writing to the Contracting Officer. The contract manager must be able to read, write, speak, and understand English or must have a qualified translator. The Contractor shall provide a phone number where the point of contact can be reached for issues regarding the delivery and condition of the vehicle. Acceptance/Inspection - An inspection will be performed on the vehicle and all vehicle modifications before the vehicle will be accepted by the Government. This inspection will show the overall condition of the vehicle. Discrepancies must be fixed before putting the vehicle in service if use of the vehicle would aggravate the problem or if the discrepancy creates a safety hazard. The 65th Logistics Readiness Squadron and 65th Civil Engineering Squadron will identify personnel to complete the joint acceptance inspection. The Contractor shall provide the specified vehicle and all services required to keep the vehicle in a safe, operational condition prior to the Acceptance Inspection. The vehicle shall be delivered upon Contractor notification by the Contracting Officer and must meet the minimum acceptance criteria specified in this Section. The Contractor shall carry insurance to cover the cost for replacement or repair of vehicles, lost, stolen, or damaged through criminal acts, natural acts (Acts of God), or hostile acts until the purchase is complete. This is to preclude the Government from being held liable for any claims. The Contractor shall provide the complete maintenance history of the vehicle. Minimum Standards: The following minimum criteria shall be met at the time of the Acceptance Inspection. The vehicle provided by the Contractor shall not be older than 12 years old regardless of the vehicles condition. The vehicle provided by the Contractor shall have less than 85,000 miles. The vehicle shall have a minimum capacity of 15 cubic meters. The vehicle shall have a fully functional ?comb? type roll bar container-handling device for 120 to 1,100 liter European containers and an ?arm? type roll bar for 700, 1,000, and 1,100 liter European containers. Both the exterior and interior of the vehicle shall be free of rust and damage. The vehicle shall be of good quality and in safe operating condition and shall comply with all local regulations applicable to this vehicle. The Government will accept or reject the vehicle promptly after receipt. If the Contracting Officer determines that the vehicle is not in compliance with this contract, the Contracting Officer will inform the Contractor in writing. If the Contractor fails to replace the vehicle or correct the defects as required by the Contracting Officer, the Government may contract for a replacement vehicle. Safety Items: The vehicle shall meet all local safety requirements and have, as a minimum, functional seatbelts, side mirrors, windshield wipers, parking brake, and horn. The vehicle shall have two brackets: one for a Portuguese plate, provided by the Contractor, and one for a USFORAZ plate to be provided by the Government. *** The system sometimes changes the dashes (-) to question marks (?).
