- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, RMK0E040084, is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 2001-03. The North American Industry classification (NAICS) code is 624310 and the business size maximum is $5.0 Million. The proposed contract is set aside for small businesses. All responsible small business sources may submit a quote, which will be considered by the Government. This office is requesting quotes on the following item: 0001: Provide all travel, supplies, materials, and personnel to provide 20 Psycho-Educational Assessment services to the BIA, Sherman Indian High School, Riverside, California in accordance with the Scope of Work (SOW). The QUANTITY is One (1), The UNIT is Lump Sum. The Government intends to award a firm fixed price contract. The period of performance is from Date of Award through December 31, 2004. The SCOPE OF WORK (SOW) is as follows: Scope of Work. SCOPE: Provide all labor (including a certified school psychologist), travel, materials, supplies, tests, etc. to provide psycho-educational assessments for Special Education(SPED) students. A confidential written assessment for each student shall be provided, which shall include, but not limited to, what assessment instruments were utilized to provide measurements on intellectual academic, visual-motor/perceptual, social/emotional behavioral functioning with a summary and recommendation for the multi-disciplinary team. The psychologist shall be available for testing of students during both school hours and non-school hours, that is, into the evening hours, after dinner time. The school psychologist shall schedule dates and times to conduct testing. Additional time shall be provided if need to complete a written assessment of each student. The school psychologist shall be responsible for all labor, testing/assessment materials and equipment need to test students. Twenty psycho-educational assessments shall be conducted. Please include your TAX ID Number, CAGE Number (obtain at www.ccr.gov), and DUNS Number when submitting your quote. The Offeror shall provide the lump sum cost for the base and four option year periods with a breakdown of their cost to provide this service and provide a copy of their certification. The offeror shall have a favorable screening (applicant and suitability) determination must be made by the Bureau of Indian Affairs (BIA) Office of Indian Education Programs (OIEP), Personnel Security and Suitability Program prior to contract award.
