- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, RMK0E040083, is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 2001-03. The North American Industry classification (NAICS) code is 624310 and the business size maximum is $5.0 Million. The proposed contract is set aside for small businesses. All responsible small business sources may submit a quote, which will be considered by the Government. This office is requesting quotes on the following item: 0001: Provide all travel, supplies, materials, and personnel to provide mental health therapist services to the BIA, Flandreau Indian School, Flandreau, SD 57028 in accordance with the Scope of Work (SOW). The QUANTITY is One (1), The UNIT is Lump Sum. The Government intends to award a firm fixed price contract. The period of performance is from August 30, 2004 through May 19, 2005 with four-option year periods. The SCOPE OF WORK (SOW) is as follows: Scope of Work. SCOPE: Students at the Flandreau Indian School come from varied and often socially, physically or emotionally traumatic background. These students are in need of immediate intervention by a Mental Health Therapist. This position will be responsible to provide services that include psychology, physical and emotional evaluations family therapy, counseling, anger management counseling/care and provide consultation. BACKGROUND: Students are in need of evaluation, counseling and treatment. OBJECTIVES: To provide psychological, physical and emotional evaluations, family therapy counseling, anger management counseling/care and provide consultation to ensure students well being. CONTRACTOR TASKS: The contractor will counsel and evaluate students at the Flandreau Indian School. The contractor shall be a Licensed Practicing Counselor. A Chemical Dependency Counselor, DC, Level 3 is preferred by not required. CONTRACT END ITEMS: Services shall start on August 30, 2004 and May 19, 2005, excluding the Christmas break which starts December 17, 2004 through January 3, 2005. Services shall be provided for two days a week at 36 weeks or 576 hours. Documentation of the Counseling session will be placed in the students file within 48 hours of the student?s counseling session. The Offeror shall provide the lump sum cost for the base and four option year periods with a breakdown of their cost to provide this service and provide a copy of their certification. The offeror shall have a favorable screening (applicant and suitability) determination must be made by the Bureau of Indian Affairs (BIA) Office of Indian Education Programs (OIEP), Personnel Security and Suitability Program prior to contract award.
