- The Contractor shall provide all labor, equipment, materials and supervision necessary to inspect and clean cooking-related exhaust systems located in fifteen (15) buildings at the United States Military Academy (USMA), West Point, New York. The Contractor shall perform services in accordance with National Fire Protection Association (NFPA) Code 96 - Ventilation Control and Fire Protection of Commercial Cooking Operations. Services shall include, but not be limited to the following: Cover all stationary equipment under and adjacent to the exhaust systems being cleaned, Inspect and clean all ducts, louvers, fans, exhaust hoods, filters and building surfaces where systems vent to the outside, Disassemble, inspect, clean and re-assemble all components associated with the cooking related exhaust systems, Remove coverings, clean and wipe equipment and wash floors to remove any grease or associated cleaning debris, and Ensure the work area is left clean. Work Hours: The Contract shall perform all services during normal work hours (8:00 AM - 4:00 PM), Monday through Friday, excluding Federal holidays, except at noted below. COOKING-RELATED EXHAUST SYSTEMS - NOTE THE TIME CONSTRAINTS FOR EACH BUILDING Superintendent??????s Quarters Not before 8:00 AM. Post School 8-4, M-F. Golf Clubhouse 8-4, M-F. Child Development Center 8-4, M-F. Natural Bridge 8-4, M-F. Camp Buckner Cadet Mess 8-4, M-F. Camp Buckner Barth Hall 8-4, M-F. Snack Bar After 2:00 PM. UPH Dining Hall After 6:00 PM. First Class Club 8-4, M-F. West Point Club 8-4, M-F. Ike Hall Snake Bar 8-4, M-F. Cadet Mess Hall After 7:00 PM. Keller Army Hospital After 6:30 PM. Burger King After 7:00 PM. Period of Performance: October 1, 2004 through September 30, 2005, with four (4) option years. Preliminary Submittals: The Contractor shall submit catalog cuts and/or descriptive literature on the cleaning agent(s) that are proposed to be used. This literature shall be submitted to the Contract Monitor within fifteen (15) days prior to the anticip ated start of services. The Contracting Officer will provide written approval within seven (7) days. Schedules: The Contractor shall coordinate inspection and cleaning schedules with the customer a minimum of fifteen (15) days prior to the anticipated start of services. The Contractor shall submit a tentative itinerary of dates and times of scheduled in spection and cleanings listed by building number to the customer and Contract Monitor. NOTE: Construction and maintenance projects are ongoing throughout USMA. Buildings may be temporarily or permanently removed from this contract at the convenience of the United States Government. Identification: The Contractor shall provide an identification badge for each employee performing services under this contract. The identification badges shall contain the Contractor??????s name, employee??????s name, contract number and project descript ion. Each Contractor employee shall conspicuously display their identification badge on their person while performing work under this contract. Safety: The Contractor shall perform all services in a safe manner. The Contractor shall notify the Contract Monitor immediately when safety or hazardous situations are confronted. Inspection Reports: The Contractor shall submit a written report to the Contract Monitor within a minimum of fifteen (15) days following the completion of any services performed. The Inspection Report shall document the following: 1.Services performed, 2.Location of services performed, 3.Condition of system prior to cleaning, and 4.Discrepancies or problems noted during inspection or cleaning. Invoices: The Contractor shall submit informational invoices to the Contract Monitor with the following information: 1.Contractor name, address and phone number, 2.Invoice date, 3.Invoice number, 4.Contract number, 5.Line item billed against (ie: 0001), 6.Amount bi lled per line item, and 7.Services performed.
