Description

IV Home Infusion Service for the Department of Veterans Affairs Medical Center (herein known as KCVA), 4801 Linwood Blvd., Kansas City, Missouri 64128. The KCVAMC will supply physician order for home infusion, medicine, related IV equipment and supplies. The KCVA will supply the medications (i.e. antibiotics, related IV equipment, supplies and appropriate physician order for home infusion. The Contractor will prepare the medication and deliver to the patient. Offeror shall educate patient on the medication, potential side effects (administration of medicines) and how to use the equipment. Potential contractor must be physically located within the Kansas City metro area as the VA will furnish medicine and related IV infusion equipment and supplies to the vendor for use in preparation and delivery to the patient. Services are to be provided for a period of one (1) year from date of award with the option to renew the contract for four (4) additional one (1) year periods. Anticipated issue date of Solicitation is August 13, 2004. Requests may be made via fax or email.
