Description

RFQ R3-WZ-04-78 DECKING MATERIAL THIS AMENDMENT CHANGES THE DELIVER TO READ AS FOLLOWS: MESA RANGER DISTRICT GOLDFIELD ADMIN SITE, 1910 BUSH HWY MESA, AZ. 85215. CHANGE THE QUANTITY AND UNIT OF ISSUE TO READ 17,280 LF This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The USDA Forest Service, Mesa Ranger District requires Trex? Decking Material for repairing docks and to match existing material. Material shall meet the following requirements. . Line Item 1. 17,280 Planks EA; SCOPE Size of Trex? decking planks is 2 inch by 6 inch by 16 feet (1.5?x5.5?x16); Color is Winchester Grey; Texture is smooth; DELIVER LOCATION is FOB Destination to USDA Forest Service, Mesa Ranger District, Goldfield Admin site, 9210 Bush Highway, Mesa AZ 85215; Directions to delivery site is form Highway 60 go north on Power Road, Power Road will turn into Bush Highway, Take left at the first four way stop intersection located on Bush Highway, after turning left go about one mile around a bend in the road, site is first right after bend. PACKAGING planks shall be packaged to accommodate offloading using a forklift. The Government will off load using a large forklift. DELIVERY SCHEDULE is 14 calendar days after receipt of order, Vendor shall provide Government 24 hour notice prior to delivery so arrangements can be made for personnel and off loading equipment, delivery can be made 7 days a week between the hours of 8:00 AM to 4:00 PM.; The NAICS CODE is 444190 with a small business size standard of annual gross receipts of $6 million dollars. CLAUSES: The provision 52.212-1, Instructions to Offerors-Commercial applies to this acquisition. The provision 52.212-2, Evaluation-Commercial Items applies to this acquisition. Evaluation will be based on past performance and price. Include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items with offers.
