Description
This is a combined synopsis/solicitation for commercial items or services IAW with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. This solicitation is issued as a Request for Quote (RFQ) No. F6334304. The solicitation clauses and provisions are those in effect through Federal Acquisition Circular 97-27. The NAICS Code is 541990. See Note No. 1 for this procurement. The Food and Drug Administration intends to award a competitive purchase order to purchase linen services for the Center for Food Safety and Applied Nutrition. Vendor shall perform the following statement of work: BACKGROUND INFORMATION: The Center for Food Safety and Applied Nutrition (CFSAN) of the Food and Drug Administration (FDA) must provide protective clothing for its laboratory personnel in order to (1) comply with good laboratory practices (GLPs) requirements, (2) meet personnel safety needs and (3) insure that important scientific efforts are not contaminated. For approximately thirty (30) years these goals have been accomplished by contracting with local laundries to provide appropriate apparel and other items needed in the laboratories. The current contract for such services expires on September 28, 2004. Therefore, arrangements must be made to continue such services, which are vital in the daily laboratory operations. OBJECTIVE: To provide protective garments and floormats needed in conducting laboratory work to CFSAN laboratory and support personnel. SCOPE OF WORK: Independently and not as an agent of the Government, the contractor shall furnish the necessary personnel, materials, services, facilities, and otherwise do all things necessary for or incident to the performance of the work as described: 1) The contractor shall pick up and deliver required laundry to the designated Food and Drug Administration facilities listed : a) College Park, 5100 Paint Branch Parkway, College Park, MD 20720-3825; b) MOD I, 8301 Muirkirk Road, Beltsville, MD 20705; c) Chantilly, VA, 3650 Concorde Parkway, Chantilly, VA 20151-1129 (Ending Date:10/17/04) (Start Date: 10/18/04) University Station, 4300 River Road, College Park, MD 20720. The contractor shall abide by all regulations pertaining to delivery, parking and entrance to the above FDA facilities. 2) The contractor shall pick up dirty laundry within the CFSAN facilities between the core working hours of 9:00 a.m. and 3:00 p.m., on the specified days of each week, excluding Federal holidays. 3) The contractor shall provide clean and freshly laundered neatly pressed coats, with a minimum of wrinkles. 4) Even though sizes are specified, the lab coats shall be generic to both males and females. 5) The contractor shall keep all articles in complete repair, which includes, but is not limited to darning, mending any holes, and replacing lost buttons. 6) The contractor shall routinely replace worn out apparel due to normal wear and tear without charge to the government. 7) The contractor shall replace and charge for articles lost or damaged beyond repair through the fault or neglect of Government personnel. The government should be notified of such articles as soon as possible. 8) The contractor shall provide all coats with personalized names. 9) The contractor shall press on the Food and Drug Administration Seal on all items. Charge should be included in prices for items. The seals will be provided by the linen service. 10) The total estimated quantities may vary plus or minus 10% due to the gain and loss of government personnel. The government reserves the right to make changes in individual quantities of types and sizes of article as may be necessary to keep laboratory personnel properly supplied. No minimum quantity of any item specified is guaranteed under the terms of this contract. 11) The contractor shall furnish necessary laundry bags and rack for soiled linen. 12) The Project Officer is the ONLY AUTHORIZED person who can request changes in the type of garments, quantities ordered and sizes. 13) The contractor shall provide occasional emergency deliveries within 24 hours after telephone notification by the Project Officer. 14) The Contracting Officer is the only person authorized to make monetary changes to this procurement. Vendor shall provide the following deliverables: The contractor shall pick up dirty laundry and deliver clean linens to the designated contact persons at the CFSAN facilities between the core working hours of 9:00 a.m. and 3:00 p.m., Monday of each week, excluding Federal Holidays. The contractor shall deliver the required supplies and services only to the Government authorized contact or alternate persons who will be identified upon award of this order. Period of Performance: Work shall commence September 30, 2004 and end September 29, 2005 ESTIMATED SUPPLIES AND QUANTITIES: 1) Coats, Laboratory, White Knee length, Herringbone twill; Sanforized, long-Sleeve, belted back, buttoned front, Three pocket (two regular, one breast) Size Range: 30-56 (Quantity: 1100); 2) 4x6 Floor Mats, reddish-black (Quantity: 3); 3) Personalized iron on name tags (Quantity: 1100) Charge (if any); 4) FDA Heat Seal Emblems (Quantity: 1100); 5) Lost Garment & Replacement Charge (Quantity: 5) The following is a listing of laboratory coat sizes required of the new contract: WOMEN: Size 30/Qty: 5; Size 32/Qty: 12; Size 34/Qty: 44; Size 36/Qty: 56; Size 38/Qty: 119; Size 40/Qty: 45; Size 42/Qty: 45; Size 44/Qty: 53; Size 46/Qty: 20; Size 48/Qty: 6; Size 50/Qty: 14; Size 52/Qty: 7; Size 54/Qty: 6; Total: 432. MEN: Size 36/Qty: 25; Size 38/Qty: 74; Size 40/Qty: 98; Size 42/Qty: 125; Size 44/Qty: 144; Size 46/Qty: 95; Size 48/Qty: 70; Size 50/Qty: 19; Size 52/Qty: 4; Size 54/Qty: 4; Size 56/Qty: 10; Total: 668 This is a formal request for quotes. However, the evaluation of other offers and any award decision will be made on a best value basis. Past Performance will be evaluated based on information provided by the offeror as to actual sales of the proposed service to industry or government agencies. Past performance will consider the ability of the items to meet technical specifications as advertised by the offeror, past record of meeting delivery schedules and service reputation. Offerors must include records of three recent sales and identify a point of contact for each by providing a name and telephone number. The government will award a purchase order to the offeror whose offer conforms to this solicitation and will be most advantageous to the government, price and other factors considered. The FDA intends to make an award immediately after the response date of this notice. The following FAR clauses shall apply to the solicitation: 52.212-1 Instructions to Offerors-Commercial; 52.212-2 Evaluation-Commercial Items; 52.212-3 Offeror Representations and Certifications; 52.214-4 Contract Terms and Conditions-Commercial Items; and 52.232-33 Electronic Funds Transfers.
