- Synopsis: COMMERCIAL FORKLIFT SOL RFQ-R5-13-04-113 DUE 09-13-04. This is a combined synopsis/solicitation for commercial item prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotation RFQ-R5-13-04-113 and offers are to be received by 4:30 p.m., 9/13/04. Questions may be directed to Nancy Ruggeri at 559-784-1500, Ext. 1240. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24. If you are a contractor under GSA contract, include the contract number and whether or not you are quoting the contract price. Following is description of the item including minimum requirements. Nissan Forklift Model PL70LP, 7000 lb lift capacity with 14 ft side shift mast, pneumatic tires, LP gas motor, light package for roading, 33# LP tank, back up alarm, micro lock park brake system, and automatic transmission or equal. Delivery shall be made to the Porterville Air Attack Base, 2125 S. Newcomb, Porterville CA 93257. Coordinate delivery with John Silva at 559-784-1500 ext 1149. Equipment shall be serviced with all fluids full and ready for operation upon delivery. Acceptance shall be made at destination. Provide best delivery schedule, but no later than November 15, 2004. Specification sheet shall be submitted with your offer that provides sufficient information for the Government to determine if the forklift offered meets all Government requirements. This acquisition is set aside for small business concerns. NAICS Code 423830. Small Business Size Standard: 100 employees. The following provisions as applicable are incorporated by reference with the same effect as if they were in full text: (1) FAR 52-212-1 Instructions to Offers?XCommercial Items. (2) FAR 52.212-2 Evaluation?XCommercial Items. The following factors shall be used to evaluate offers: technical capability of the item offered to meet the Government??s requirements; past performance; and price. Technical and past performance when combined are of equal importance when compared to price. (3) FAR 52.212-3 Offeror Representations and Certifications?XCommercial Items. (4) FAR 52.212-4 Contract Terms and Conditions?XCommercial Items. (5) FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes of Executive Orders?XCommercial Items, which includes 52.219-6, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-3, and 52.232-33. (6) Offerors shall provide completed copies of FAR 52.212-3 (Offeror Representations and Certifications?XCommercial Items) with their quote. The Offeror is required to provide their Contractor??s DUNS Number and required to be registered in the Central Contractor Registration database (CCR).
