Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, RMK0E040120, is issued as a Request for Quotes (RFQ) under test procedures of FAR 13.5. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 2001-03. The North American Industry classification (NAICS) code is 62134 and the business size maximum is $5 Million. This requirement is set aside for small businesses. This office is requesting quotes on the following items: 0001 Provide all travel, supplies, materials, and personnel to provide occupational therapy services to Tuba City Boarding School, Tuba City, Arizona during the school year 20042005 in accordance with the Scope of Work (SOW). The QUANTITY is One (1), The UNIT is Lump Sum. Period of Performance (POP): September 7, 2004 to June 30, 2005. 0002 Option Year One Provide all travel, supplies, materials, and personnel to provide occupational therapy services to Tuba City Boarding School, Tuba City, Arizona during the school year 20052006 in accordance with the Scope of Work (SOW). The QUANTITY is One (1), The UNIT is Lump Sum. Period of Performance (POP): July 1, 2005 to June 30, 2006. 0003 Option Year Two. Provide all travel, supplies, materials, and personnel to provide occupational therapy services to Tuba City Boarding School, Tuba City, Arizona during the school year 20062007 in accordance with the Scope of Work (SOW). The QUANTITY is One (1), The UNIT is Lump Sum. Period of Performance (POP): July 1, 2006 to June 30, 2007. 0004 Option Year Three. Provide all travel, supplies, materials, and personnel to provide occupational therapy services to Tuba City Boarding School, Tuba City, Arizona during the school year 20072008 in accordance with the Scope of Work (SOW). The QUANTITY is One (1), The UNIT is Lump Sum. Period of Performance (POP): July 1, 2007 to June 30, 2008. 0005 Option Year Four. Provide all travel, supplies, materials, and personnel to provide occupational therapy services to Tuba City Boarding School, Tuba City, Arizona during the school year 20082009 in accordance with the Scope of Work (SOW). The QUANTITY is One (1), The UNIT is Lump Sum. Period of Performance (POP): July 1, 2008 to June 30, 2009. Please provide a breakdown of your costs. The Government intends to award a firm fixed price contract with a base and four option year periods. SCOPE OF WORK: The contractor shall provide evaluations; screening for early intervention and as part of Child Find; provide direct therapy services; train school wide staff and parents. The contractor will provide occupational therapy services to about 10-15 occupationally impaired students either previously identified as needing therapy services or those who may enter the program through out the school year 2004-2005. Contract will provide re-evaluation on three students in 2004-2005. Students shall be fully evaluated according to strict Federal and Arizona State guidelines. Tests and other evaluation materials used to assess a child shall be selected and administered so as not to be discriminatory o a racial or cultural basis. OCCUPATIONAL THERAPY. 1. The registered occupational therapist will complete evaluations and consult with the certified occupational therapy assistant concerning treatment programs. 2. Contractor shall develop a schedule for the evaluations and placement of students; the schedule shall be developed with input from the Special Education Department, Tuba City Boarding School. 3. The Contractor shall evaluate and prepare a summary report for each individual students file. The summary report shall be completed and available for student Multi-Evaluation Team meeting as scheduled. 4. Recommendations shall be made to the special education teacher and the regular program classroom teacher on a consultant basis. 5. The contractor shall provide training and instructions to a paraprofessional staff member of the school. Training will enable the paraprofessional to deliver the appropriate direct services to students during the time between scheduled visits by the certified occupational therapist. 6. The contractor shall develop an IEP, which will outline occupational therapy services delivered on both a consultant and director therapy services basis. DIRECT THERAPY. 1. Contractor shall provide direct therapy services individually or in small group settings, as appropriate. 2. Recommendations shall include adaptive equipment to enable optimum classroom performance. 3. Appropriate services according to the students IEP will guide direct therapy services. 4. The contractor shall attend all IEP meetings. 5. Contractor shall prepare one IEP that will include other services the student is receiving in other disability area(s). 6. Contractor will provide Extended School Year Services for eight (8) weeks during summer break if needed. 7. The contractor shall attend one parent information meeting. 8. Contractor shall attend at least one of the two contract providers meetings with the Western Navajo Agency, Office of Indian Education Programs, Special Education Coordinator. 9. Contractor shall submit a monthly - summarized invoice. Invoice shall contain the following information. a. Purchase Order Number b. Date services rendered c. Name of technician providing services d. Description of services rendered e. Time spent with student (hours, minutes, etc.) 10. The required service days per month: Six and one half (6.5) hours direct student service at 2 visits per week times 8 visits per month. 11. Contract period is from date of award though June 30, 2005 with four option years. 12. Location of Service Site: Tuba City Boarding School, Tuba City, Arizona. QUALIFICATIONS OF EXPERT PROVIDER: Knowledge, skills and abilities required of expert provider: 1. Provider must have an occupational therapy license to practice in the State of Arizona. 2. Provider must experience working with Native American Indian Children. Closing Date of Quote is September 2, 2004 9:00am local time (Albuquerque, New Mexico).
