Description

Subject solicitation is amended as follows: 1. Change offer due date to read "8/31/04" in lieu of "8/17/04." 2. Page 6, Change heading to read "Estimated Quantity" in lieu of "Estimated Quantity per Month" for the base and all option years. 3. Amend item #1 for the base and all option years to read "Concentrator with backup system set-up consisting of compressed gas source, regulator, stand, (nasal cannula or mask and humidifier when specified) and disposable supplies. Concentrator will have flow rate capacity up to 5 LPM. Cylinder size M or H." 4. Delete item #2 in it's entirety for the base year and all option years. 5. For items #1 - 12 and #14-17 all quantities are to be multiplied by 12 for the base and all option years. 6. For item #13 for the base and all option years, change the quantity to read "5,000 lbs." 7. Any mention of size 6 LPM shall be changed to 5 LPM. 8. Insert revised pages 6 through 17 which reflects the above changes. NOTE: THIS NOTICE MAY HAVE POSTED ON WWW.FEDBIZOPPS.GOV ON THE DATE INDICATED IN THE NOTICE ITSELF (05-AUG-2004).
