Description

Contractor shall provide to the VISN 2 Medical Centers located in Albany, NY; Syracuse, NY; Bath, NY; Canandaigua, NY; Western, NY (Buffalo, NY & Batavia, NY) services for the delivery of home oxygen to VA beneficiaries. The contract period is 11/1/04 through 10/31/05 with two option years and one incentive year for excellent service, all subject to availability of funds. Offerors are advised that they are responsible for obtaining all amendments. Interested parties are required to submit prices and clear and convincing data substantiating their qualifications to satisfy this Request for Proposal. Offers are due by 4:00 PM on 9/28/04 and should be mailed to the Contracting Officer at Department of Veteran's Affairs Medical Center, 3495 Bailey Ave. Buffalo, NY 14215, Attention: Mary T. Betker, Contracting Officer (528 - NCA). Telephone requests and e-mail requests will not be honored. Electronic offers will not be accepted. This solicitation is Cascaded to allow opportunities for Small Disabled Veteran Owned Businesses, Veteran Owned Businesses, Small Businesses (including 8A and Women Owned), and full and open competition.
