- This is a combined synopsis/solicitation for non-personal services prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; propo sals are being requested from all sources. No formal solicitation will be issued but a statement of work follows below. This is an Indefinite Quantity Contract (IDIQ), Firm Fixed Price acquisition with a basic 12 months period of performance and 3 option s for 12 months each. This IDIQ contract will allow SMDC to potentially offer its contract to other DOD and federal agencies with an individual Delivery Order for each agency. The objective is to provide the US Army Space and Missile Defense Command (SMDC ) and other DOD agencies commercial fitness facilities for approximately three hundred (300) members (numbers depend on different Delivery Order issue under this effort). August 24, 2004 STATEMENT OF WORK COMMERCIAL FITNESS. I. In order to ensure the stre ngth and stamina of SMDC personnel to complete their respective missions successfully, the Department of Defense provides necessary physical fitness and training areas and facilities. Maintaining the physical fitness of SMDC personnel is essential to kee ping a healthy force ready for combat efficiency and work performance in defense of the United States of America. Military fitness standards are outlined in AR600-63, 215-1 and 5 U.S.C. 7901, respectively. In accordance with AR600-63, AR 215-1 fitness fac ilities contribute to US Army readiness by enhancing the physical and mental stamina of military and civilian personnel. This is accomplished by providing a variety of programs that contribute to fitness. II. SCOPE: The contractor shall provide membership benefits to US Army Space and Missile Defense Command and other DOD agencies whose official or TDY assignment is located only at Crystal City, Arlington, VA. Eligible participants are defined as all active duty and civilian personnel whose duty station is in the Crystal City area in Arlington, VA. III. MINIMUM REQUIREMENTS OF THE PHYSICAL FITNESS FACILITY. In order to support the US Army Space and Missile Defense Command mission effectively, the contractors fitness staff must be knowledgeable of preventat ive health maintenance and weight management. The contractors facility must offer core fitness programs that contribute to cardiovascular fitness, strength conditioning, and flexibility, i.e. total fitness. As a minimum, the following fitness programs/e quipment must be offered: cardiovascular equipment, strength conditioning equipment, aerobic classes and walking and jogging programs. The commercial fitness facility shall be located within 1 mile of SMDC (1601 South Bell St). IV. FULL MEMBERSHIP BENEF ITS A. Membership in the fitness facility shall include: 1. All standard commercial services offered by the fitness facility, 2. Unlimited access during normal operating hours, and 3. Additional discounts normally extended to members. B. Membership do es not include: 1. The use of health spas, massages, racquet clubs, and golf courses, bowling centers, sporting clubs, and swimming facilities unless included in a standard commercial membership. C. The rights to guest passes and membership privileges s hall belong to SMDC and other identified DOD agencies as represented by the Agency Fitness Program Manager, and not to individual participants. V. MEMBERSHIP CARDS. Membership cards issued by the contractor shall identify the name of the government part icipant along with his or her unit. Any replacement fee for lost cards is the responsibility of the participant. VI. PROCEDURES A. Memberships shall run from the first day of the month for a 12-month period. The member will register through SMDC or their respective DOD agency point of contact. Each customer will review the membership rules and submit application through an e-mail NLT 25th of each month t o be eligible for the following month. The Agency Fitness Program Manager will issue commercial membership cards given to them by the contractors manager. B. Once the member has registered and is approved, the respective Agency Fitness Program Manager w ill issue membership cards to each group. The contractor shall submit a monthly members usage report NLT five days from the end of that month to the Agency Fitness Program Manager. (categorized by Delivery Order or Task Order number). If the member elect s to initiate membership prior to the first of the next month without approval from the Agency Fitness Program Manager, then the member shall be responsible for any fee charged. C. TDY personnel must follow the same rules and application process to be gran ted a temporary membership card. The fitness facility shall submit invoice to the Agency Fitness Program Manager. Invoices for memberships must be identified in a monthly invoice with a listing of members names or card numbers. The Government shall no t be responsible for making a payment until the Agency Fitness Program Manager receives a completed list by name or card number with the invoice of services. D. The contractor shall submit a monthly invoice to the Agency Fitness Program Manager including regular membership and TDY member usage report. The Government shall not be responsible for making a payment until the Agency Fitness Program Manager receives a completed list by name with the invoice of services. E. The contractor shall submit an invo ice for membership cards on monthly basis; The Agency Fitness Program Manager will manage membership cards. If less than 15 people per agency choose to continue membership during a given month, those memberships will be placed on inactive status. Additio nal membership may be granted. The fitness facility will need to submit an invoice for additional members beyond the first 15. F. Participant memberships may be terminated upon 30 days notice with termination effective at the end of the following full-cal endar month. Additional funds due the Government resulting from membership termination shall be credited with in-active status of membership card. Anyone able to provide the aforementioned services is encouraged to respond. If you are selected, you must be registered in the Central Contractor Registration (CCR) (http://www.ccr.gov/) in order to receive the award. EVALUATION PROCESS FOR AWARD: Award will be made to the technically acceptable, lowest priced quotation from a responsible Quote as defined in F AR Part 9, who also has satisfactory record of past performance. Evaluation Factors are as follows: 1) Technical Approach. Request you prepare a Technical Approach that includes information that demonstrates your technical capability and proven past perf ormance to successfully complete the requirement as outlined in SOW Section III (Maximum of five (5) pages); 2) Execution Plan: Illustrate or discuss the project execution, example the issue of membership cards, an updated computer system capable of mainta ining separate membership for each government agency, and printing a report monthly (Maximum of five (5) pages); and 3) Past Performance: submit three (3) references of similar work performed in the past three years. References should include contract n umbers, performance periods, and point of contact with phone numbers. Past Performance elements to be reviewed include quality, delivery, cost control, and customer service. Price: submit a firm-fixed-price quote for the effort outlined in the Statement of Work. Price should be clearly stated for full month, and/or annual membership. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-21.
